超声冲洗在慢性牙髓炎一次性根管治疗术中的应用

Application effect of ultrasonic irrigation on chronic pnipitis one-time root canal therapy

目的 探讨超声冲洗在慢性牙髓炎一次性根管治疗术中的应用效果。 方法 抽取慢性牙髓炎患者84例，通过随机数字表法分为对照组和研究组，每组42例。两组均采用一次性根管治疗术治疗，对照组给予常规注射器冲洗，研究组给予超声冲洗。1年后随访，统计并对比两组临床疗效、术后疼痛发生情况，并分析治疗前后两组炎症因子[白细胞介素-6（IL-6）、白细胞介素-8（IL-8）、肿瘤坏死因子-α（TNF-α）、高敏C-反应蛋白（hs-CRP）]水平变化情况。 结果 研究组痊愈率（95.24%）与对照组（90.48%）比较，差异未见统计学意义（P〉0.05）。研究组疼痛发生率（9.52%）低于对照组（28.57%），差异有统计学意义（P〈0.05）。研究组侧支根管填充率（23.81%）高于对照组（4.76%），差异有统计学意义（P〈0.05）。治疗前两组IL-6、IL-8、hs-CRP、TNF-α水平比较差异未见统计学意义（P〉0.05）；治疗后两组炎症因子水平均较治疗前降低，且研究组IL-6、IL-8、hs-CRP、TNF-α水平低于对照组，差异有统计学意义（P〈0.05）。 结论 慢性牙髓炎一次性根管治疗术中给予患者超声冲洗效果显著，可有效降低炎症因子水平及疼痛发生率，并提高侧支根管填充率，具有推广价值。

Objective To investigate the application effect of ultrasonic irrigation on chronic pulpitis one-time root canal therapy. Methods Eighty-four cases of chronic pulpitis were selected and divided into control group and study group according to random number table, with 42 cases in each group. Two groups were both treated by one-time root canal therapy, but the control group was given con- ventional syringe irrigation, the study group was given ultrasonic irrigation. Follow up after 1 year, and the clinical curative effect and occurrence of pain after operation of the two groups were compared ; the in- flammatory cytokines [ interleukin -6 （ IL-6）, interleukin-8 （ IL-8 ）, tumor necrosis factor alpha （ TNF- alpha）, high sensitive C reactive protein （hs-CRP） level change] of the two groups before and after treatment were analyzed. Results The difference of cure rate between the study group （95.24%） and the control group （90. 48% ） was not statistically significant （P 〉 0.05 ）. The incidence of pain in the study group （9.52%） was lower than that in the control group （28.57%）, and the difference was statis- tically significant （P 〈 0.05 ）. The root canal filling rate of the study group （23.81% ） was higher than that of the control group （4. 76% ）, and the difference was statistically significant （P 〈0. 05）. Before treatment, the differences of IL-6, IL-8, hs-CRP, TNF-α levels between the two groups had no statistic significance （P 〉 0. 05 ） ; after treatment, the levels of inflammatory factors of the two groups were lower than before treatment, and the levels of IL-6, IL-8, TNF-α and hs-CRP in the study group were lower than those in the control group （ P 〈 0.05）. Conclusions The efficacy of ultrasonic irrigation in chronic pulpitis one-time root canal therapy is significant, can effectively reduce the incidence of pain and inflam- matory cytokines levels, and improve the lateral canal filling rate, and is worthy of recommendation.
