生脉注射液治疗心肺复苏后心功能不全(气阴两虚证)的临床观察

Clinical Observation of Shengmai Injection on Cardiac Insufficiency after Cardiopulmonary Resuscitation(Syndrome of Deficiency of both Qi and Yin)

目的观察生脉注射液治疗心肺复苏后心功能不全(气阴两虚证)的临床疗效并探讨其机制。方法将60例心肺复苏后心功能不全(气阴两虚证)患者随机分为治疗组及对照组,各30例。对照组予亚低温脑保护、呼吸机辅助呼吸、维护器官功能、血液净化等常规治疗。治疗组在常规治疗基础上加用生脉注射液50 mL静脉推注,每日2次,疗程1周。疗程结束后比较两组患者中医证候疗效、脑钠肽(BNP)、心脏输出指数(CI)、全心射血分数(GEF)、血管外肺水指数(EVLWI)。结果与对照组相比,治疗组的中医证候改善总有效率高于对照组(P<0.05),治疗组治疗后CI、GEF明显升高,BNP、EVLWI较前下降,两组比较差异均有统计学意义(P<0.05)。结论在西医常规治疗的基础上加用生脉注射液可显著改善心肺复苏后心功能不全患者的血液动力学指标,增强其心功能,且未见明显不良反应,值得临床进一步研究。

Objective： To observe the clinical effect of Shengmai Injection on patients with cardiac insufficiency after cardiopulmonary resuscitation（syndrome of deficiency of both Qi and Yin）,and to explore its mechanism.Methods： 60 cases of patients with cardiac insufficiency after cardiopulmonary resuscitation（syndrome of deficiency of both Qi and Yin） were randomly divided into the treatment group and the control group,30 cases in each. The control group received routine treatment,such as brain protection with mild hypothermia,assisted respiration,organ function maintenance,blood purification,and so on. The treatment group was treated with Shengmai Injection,50 m L intravenous injection,twice a day on the basis of routine treatment,and the course of treatment was 1 week. After treatment,TCM syndrome integral change,BNP,CI,GEF and EVLWI were Compared between the groups. Results： Compared with the control group,the TCM syndrome score of the treatment group decreased;CI and GEF increased significantly;BNP and EVLWI decreased Compared with before,and there were significant differences between the two groups（P 0.05）. Conclusion： On the basis of Western medicine routine treatment,Shengmai Injection can significantly improve hemodynamic indexes,and enhance cardiac function of the patients with cardiac insufficiency after cardiopulmonary resuscitation,and there are no significant adverse reactions,worthy of further clinical research.
