无保护会阴接生法与传统接生法对初产妇会阴裂伤程度、产后疼痛、产后性生活质量影响的对比研究

Effects of unprotected perineum delivery and traditional birth control on the degree of perineal laceration,postpartum pain and postnatal quality of sexual life in primipara

目的:探讨无保护会阴接生法对初产妇会阴裂伤程度、产后疼痛、产后性生活质量的影响。方法:选择2012年1月至2016年6月本院收治经阴道分娩初产妇80例,对照组实施传统经阴道分娩,观察组实施本研究无保护会阴接生技术,比较两组会阴裂伤情况、生产后3个月FSFI评分比较、生产后恢复正常性生活时间、性生活时性唤起时间以及每月性生活次数及干预不同时间点疼痛程度。结果:观察组会阴完整比例显著高于对照组(P<0.05),观察组行会阴切开比例显著少于对照组(P<0.05),观察组FSFI评分中性欲望强度、性兴奋唤起事件、性高潮出现比例、性交疼痛感受、性交时阴道湿润度及对性生活的整体满意度得分均显著高于对照组(P<0.05),观察组生产后恢复正常性生活时间显著早于对照组(P<0.05),性生活时性唤起时间显著快于对照组(P<0.05),每月性生活次数显著多于对照组(P<0.05),观察组术后当天、术后48h及出院时,入组者其疼痛视觉模拟评分均显著低于对照组(P<0.05)。结论:无保护会阴分娩接生技术能有效的减少会阴裂伤程度、提高产后性生活质量、减少产后疼痛。

Objectives： To study the effect of unprotected perineum delivery on the degree of perineal lacer- ation, postpartum pain and postnatal quality of sexual life in primipara. Methods： From January 2012 to June 2016, 80 vaginal delivery primiparas were treated in the hospital. The traditional vaginal delivery was selected as control group. The observation group received unprotected perineum delivery technique. Perineal laceration, FSFI score 3 months after delivery, time to restore normal sexual life, sexual arousal time, the frequency of sexual life and pain degree at different time points of intervention of two groups were compared. Results ： The proportion of in- tegrity perineum in observation group was significantly higher than that in control group （ P 〈 0. 05 ）. The scores of sexual desire, sexual arousal, sexual orgasm, painful sexual intercourse, vaginal wetness and sexual satisfaction in observation group were significantly higher than these in control group （P 〈 O. 05 ）. Sexual arousal time in observa- tion group was significantly faster than that of control group （ P 〈 0. 05 ）. Time of sexual arousal in observation group was significantly faster than that of control group （ P 〈 0.05 ）. The visual analogue scores of pain in observation group were significantly lower than those in control group （P 〈 0. 05 ） , and the visual analogue scores of observation group were significantly lower than those of the control group after the operation, 48 hours after operation and 48 hours after hospital discharge. Conclusion： Unprotected perineum delivery technique can effectively reduce the de- gree of perineal laceration, improve post - natal quality of sexual life, and reduce post - natal pain.
