摘要
目的:分析支气管镜下氩等离子体凝固术(APC)联合冷冻治疗中央型肺癌伴大气道阻塞疗效。方法:选取2014年12月至2016年11月我院接受治疗气道梗阻的肺癌患者120例,根据患者意愿分为对照组和观察组,每组患者各60例。对照组给予常规化疗,观察组在常规化疗基础上联合支气管镜下APC联合冷冻介入治疗。观察和评价两组患者治疗后临床效果、生存期、Karnofsky评分及安全性。结果:观察组60例患者治疗后完全有效22(36.7%)例、部分有效32(53.3%)例、总有效率高达90.0%,显著优于对照组(P<0.05)。观察组患者生存期和Karnofsky评分均高于对照组,差异有统计学意义(P<0.05)。APC联合冷冻介入治疗安全性较好。结论:支气管镜下氩等离子体凝固术联合冷冻治疗中央型肺癌伴大气道阻塞疗效较单纯化疗突出,安全性好,可延长生存期,值得临床推广应用。
Objective: Bronchoscopic argon plasma coagulation analysis( APC) combined with cryotherapy in the treatment of central type lung cancer with airway obstruction. Method: From December 2014 to November 2016,120 cases of lung cancer patients with airway obstruction treated in our hospital were divided into control group and observation group according to the patients' willingness,with 60 patients in each group. The control group was given conventional chemotherapy,and the observation group was treated with APC under the bronchoscope combined with freezing interventional therapy on the basis of routine chemotherapy. The clinical effect,survival time,Karnofsky score and safety of the two groups were observed and evaluated. Result: In the observation group,22 out of 60 cases had completely effect( 36. 7%),32 cases had effect( 53. 3%),the total effectiveness was 90%,significantly better than that of the control group( P 0. 05). The patients' survival time and Karnofsky score were significantly higher than those in the control group,the difference was statistically significant( P 0. 05). The survival time and Karnofsky score of the observation group were higher than those of the control group,the difference was statistically significant( P 0. 05). APC combined with cryoablation was safe. Conclusion: Bronchoscopic argon plasma coagulation combined with cryotherapy in the treatment of central type lung cancer with airway obstruction,when compared with chemotherapy,has more prominent curative effect,better safety and longer survival time,thus it is worthy of clinical application.
