摘要
目的探讨超声心动图测定海拔3500米以上地区,正常新生儿心脏径线值,为临床提供参考范围。方法选取175例在我院48小时内出生的正常新生儿作为研究对象,分为男婴组(n=75)和女婴组(n=100),采用彩色多普勒超声测定两组的各心腔、主动脉、肺动脉径线值,测量结果以均值()表示,进行统计学分析。通过查阅相关文献资料,将内地新生儿心脏径线值与本研究进行对比。结果男婴组的左心室、左心房、右心室、右心房、主动脉、肺动脉径线值分别为(15.4133±1.76370),(9.5200±1.79609),(13.9333±1.42690),(13.1600±1.68491),(7.9067±1.02895),(8.2133±1.17742)。女婴组的左心室、左心房、右心室、右心房、主动脉、肺动脉径线值分别为(14.6700±1.65178),(9.5100±1.76667),(13.5400±1.62319),(12.9000±2.05235),(7.6200±1.33167),(8.1500±1.26631)。两组的新生儿左心室前后径线值(LVD),差异有统计学意义(p<0.05),但左心房、右心室、右心房、主动脉、肺动脉值,差异均无统计学意义(P>0.05)。本研究与内地新生儿心脏径线值相比,差异均有统计学意义(p<0.05)。结论本研究获得的正常新生儿心脏径线值,可作为高原地区新生儿心脏径线值的参考范围,但由于样本数量太少,有待扩大样本数量进行深入研究。
Objective: To investigate the echocardiographic detection of cardiac diameters of normal neonates in plateau area for providing reference range. Methods: In total, 175 normal neonates within forty-eigh hours in our hospital were selected as the objects of study. They were divided into boy group (n=75) and girt group (n=100). The cardiac diameters were detected with by echocardiograph. Measurement data were expressed as + s for statistical analysis. In the study, the cardiac diameters were compared with mainland neonates through consulting literature materials. Results: (1) The measured values of left ventricle, left atrium, right ventricle, right atrium, aortic, and pulmonary artery in boy group were (15.4133 ± 1.76370), (9.5200 ± 1.79609), (13.9333± 1.42690), (13.1600 ± 1.68491), (7.9067 ± 1.02895), (8.2133 ± 1.17742), respectively. Meanwhile, in girl group were (14.6700 ± 1.65178), (9.5100±1.76667), (13.5400 ± 1.62319), (12.9000±2.05235), (7.6200 ±1.33167), (8.1500±1.26631), respectively. (2) There was significant difference in the measured values ofanteroposterior diameter in left ventricle between the two groups (P〈0.05). However, there were no significant differences in the measured values of left atrium, right ventricle, right atrium, aortic, and pulmonary between the two groups (P 〉 0.05). (3) Comparing with mainland newborn, there was significant difference in cardiac diameters in this study (p〈0.05). Conclusions: In this study, the cardiac diameters can be used as reference range for detecting normal neonates in plateau area. Unfortunately, due to the small number of sample, more studies with large sample are needed to be expanded.
