血清白介素6、C反应蛋白、降钙素原在卵巢囊肿蒂扭转鉴别诊断中的意义

The Clinical Value of Serum Interleukin 6, C-reactive Protein and Procalcitonin in the Identification of Ovarian Cyst Torsion

目的探讨血清白介素6(IL-6)、C反应蛋白(CRP)、降钙素原(PCT)浓度在卵巢囊肿蒂扭转鉴别诊断中的意义。方法选择2015年1—12月临床诊断卵巢囊肿病例41例,18例为卵巢囊肿蒂扭转,15例为卵巢囊肿感染,8例为卵巢囊肿破裂,比较分析血清IL-6、CRP、PCT浓度在三组病例间的差异。结果三组患者的血清IL-6、CRP和PCT水平对比差异具有统计学意义(P<0.05),两两对比结果表明,蒂扭转组血清IL-6、CRP和PCT水平均低于感染组和破裂组,对比差异具有统计学意义(P<0.05),感染组和破裂组血清IL-6、CRP和PCT水平对比差异无统计学意义(P>0.05)。结论血清IL-6、CPR、PCT是卵巢囊肿蒂扭转鉴别诊断的理想指标,可较快将卵巢囊肿破裂、卵巢囊肿感染排外,利于及时诊断及处理卵巢囊肿蒂扭转。

Objective To explore the clinical value of serum Interleukin 6（IL-6）, C-reactive protein （CRP） and proealeitonin （PCT） in the identification of ovarian cyst torsion. Methods 41 cases of ovarian cysts were diagnosed from January and December 2015, among them, 18 cases were ovarian cyst torsion, 15 cases were ovarian cyst infection, 8 cases were ovarian cyst rupture, and the serum concentration of IL-6, CRP, PCT was compared among the three groups of cases. Results The difference of serum concentration of IL-6, CRP and PCT among three groups was statistically significant （P 〈 0.05）. The concentration of IL-6, CRP and PCT in the torsion group was lower than those in the infection group and the rupture group （P 〈 0.05）. There was no significant difference in serum concentration of IL-6, CRP and PCT between the infected group and the ruptured group （P 〉 0.05）. Conclusion The serum concentrations of IL-6, CRP and PCT are ideal markers in the identification of ovarian cyst torsion. They could exclude ovarian cyst infection and ovarian cyst rupture rapidly in the identification of ovarian cyst torsion, and which could be useful in diagnosing and treating ovarian cyst torsion promptly.
