摘要
目的研究观察糖尿病合并肾病患者尿白蛋白和血清内皮素的变化情况以及对疾病诊断的临床指导意义。方法选择该院2015年1月‐2016年12月收治的糖尿病合并肾病患者150例为研究对象为观察组,另取50例同期来该院体检的健康人群为对照组。采用酶联免疫法对患者的尿白蛋白(UM)含量进行检测,并根据UM高低将糖尿病合并肾病患者分为微量蛋白尿组、中度蛋白尿组及高蛋白尿组,并采用放射免疫法检测患者的血清内皮素(ET)水平,同时对健康人群的UM和ET水平值进行检测,对以上各组患者和对照组人群的UM、ET水平值进行统计分析,观察UM、ET水平高低与糖尿病合并肾病发病程度的关系。结果糖尿病合并肾病患者的UM、ET水平值显著高于对照组(P<0.05),糖尿病合并肾病患者组内随着UM水平值的升高,ET含量也同步增加。结论糖尿病合并肾病的发病会导致患者尿蛋白和血清内皮素升高,随着病程的进展而同步加重,表明血管内皮细胞损伤参与糖尿病肾病的发生和进展。
【Objective】To observe the changes of serum albumin and serum endothelin in patients with diabetic nephropathy(DN) and its clinical significance in the diagnosis of the disease.【Methods】Totally 150 cases of diabetic nephropathy patients admitted to our hospital during January 2015 and December 2016 were selected as the research objects,and 50 healthy subjects who received physical examination in our hospital during the same period were selected as the control group.Urinary microalbumin(UM) was detected by enzyme-linked immunosorbent assay,and the diabetic nephropathy patients were divided into normal albuminuria group,microalbuminuria group and high proteinuria group according to the level of UM,and serum endothelin(ET) level was detected by radioimmunoassay.At the same time,the levels of UM and ET were detected in healthy subjects.The levels of UM and ET in the above groups were compared with those of the control group,and the relationship between the level of UM,ET and the incidence of diabetic nephropathy were observed.【Results】The levels of UM and ET in patients with diabetic nephropathy were significantly higher than those in the control group(P〈0.05).In diabetic nephropathy patients,the content of ET increased with the increase of the level of UM.【Conclusion】The incidence of diabetic nephropathy can lead to the increase of urinary protein and serum endothelin,and it will aggravate with the progression of disease course,which indicates that vascular endothelial cell injury is involved in the occurrence and development of diabetic nephropathy.
