摘要
目的:探讨咀嚼肌痉挛患者的临床及肌电图特点。方法:收集偏侧咀嚼肌痉挛患者4例进行临床资料总结,并进行咀嚼肌针电极及瞬目反射组合检查。结果:咀嚼肌痉挛患者表现为偏侧咀嚼肌阵发性不自主痉挛,多在寒冷和紧张劳累时诱发,通常患者会有张口受限及肌肉疼痛,可伴有咀嚼肌肥大。神经系统查体角膜反射、下颌反射均正常,面部针刺觉正常。4例患者针电极肌电图均可见与痉挛发作同步的群放电位,瞬目反射未见异常,1例肌电图募集相呈单纯相。结论:肌电图对于正确诊断咀嚼肌痉挛具有重要价值,本研究支持咀嚼肌痉挛起源于周围神经的学说。
Objective: To investigate the clinical and electromyography (EMG) features of patients with masticatory spasm. Method: The clinical data of four patients with masticatory spasm were summarized. The masticatory muscle needle electrode and blink reflex combination check were performed in the patients Results: The patients with masticatory muscle spasm show paroxysmal spasms of unilateral masticatory muscles, which were usually induced by cold and tense exertion, the patients usually suffer from restriction of mouth opening and muscle pain, and may be accompanied by hypertrophy of masticatory muscles. The corneal reflex, mandibular reflex and facial acupuncture sensation were normal. Needle electrode electromyography of the four patients showed grouped potentials synchronously with the onset of spasm, without abnormal blink reflex, and 1 cases showed incomplete interference pattern. Conclusion: EMG has important value for correct diagnosis of masticatory spasm. This study supports the theory that masticatory spasm originates from the peripheral nerve.
