摘要
目的探讨无创正压呼吸机辅助通气治疗慢性阻塞性肺疾病(COPD)合并呼吸衰竭疗效。方法选取我院COPD合并呼吸衰竭患者78例,依照治疗方案不同分组,各39例。对照组予以常规抗感染、解痉、止咳、化痰、吸氧等治疗,观察组在对照组基础上予以无创正压呼吸机辅助通气治疗。比较两组临床治疗效果。结果观察组治疗后动脉血氧分压(Pa O2)、血二氧化碳分压(Pa CO2)高于对照组,呼吸频率及心率低于对照组,差异有统计学意义(P<0.05);观察组治疗有效率89.74%(35/39),高于对照组66.67%(26/39),差异有统计学意义(P<0.05);观察组气管插管率10.26%(4/39),低于对照组30.77%(12/39),差异有统计学意义(P<0.05)。结论无创正压呼吸机辅助通气治疗COPD合并呼吸衰竭疗效显著,可降低气管插管率,改善患者动脉血气指标。
Objective To evaluate the efficacy of noninvasive positive pressure ventilator assisted ventilation in the treatment of chronic obstructive pulmonary disease(COPD) complicated with respiratory failure.Methods 78 cases of COPD complicated with respiratory failure in our hospital were selected,according to the treatment plan,39 cases were divided into different groups.Control group was treated with routine anti infection,antispasmodic,cough,expectorant,oxygen inhalation and other treatment,observation group was treated with noninvasive positive pressure ventilator assisted ventilation on the basis of the control group.Clinical effects of the two groups were compared.Results The arterial oxygen partial pressure(Pa O2) and the blood carbon dioxide partial pressure(Pa CO2) of the observation group after treatment were higher than those of the control group,respiratory rate and heart rate were lower than that of the control group,the difference was statistically significant(P〈0.05);effective rate of observation group was 89.74%(35/39),higher than the control group 66.67%(26/39),the difference was statistically significant(P〈0.05);tracheal intubation rate of the observation group was 10.26%(4/39),lower than the control group 30.77%(12/39),the difference was statistically significant(P〈0.05).Conclusion Noninvasive positive pressure ventilator assisted ventilation is effective in the treatment of COPD combined with respiratory failure.It can reduce the rate of endotracheal intubation and improve the arterial blood gas index.
