摘要
目的:通过检测p16^(INK4a)与Notch1基因在宫颈病变中的表达及其差异分析,筛选出灵敏度和特异性高的宫颈癌早期诊断生物标记物.方法:通过免疫组化SP法对病理学检查后的187例宫颈病变患者的石蜡组织切片进行p16^(INK4a)和Notch1蛋白表达情况检测,其中43例宫颈癌、26例重度宫颈上皮内瘤变(CIN-Ⅲ)、28例中度宫颈上皮内瘤变(CIN-Ⅱ)、30例轻度宫颈上皮内瘤变(CIN-Ⅰ)、34例子宫平滑肌瘤、26例慢性宫颈炎.结果:p16^(INK4a)蛋白在宫颈癌、CIN-III、CIN-II、CIN-I、子宫平滑肌瘤和慢性宫颈炎中阳性表达率分别为100.00%(43/43)、92.31%(24/26)、57.14%(16/28)、53.33%(16/30)、41.18%(14/34)、30.77%(8/26),差异有显著性意义(P<0.05);Notch1蛋白在宫颈癌、CIN-III、CIN-II、CIN-I、子宫平滑肌瘤和慢性宫颈炎中阳性表达率分别为83.72%(36/43)、46.15%(12/26)、57.14%(16/28)、53.33%(16/30)、44.12%(15/34)、61.54%(16/26),差异有显著性意义(P<0.05);在相同类型宫颈病变组织中p16^(INK4a)与Notch1的检测结果阳性表达率有显著性差异(P<0.05).结论:经比较分析p16^(INK4a)与Notch1两基因在宫颈病变中的表达,p16^(INK4a)基因灵敏度和特异性均良好,可以将p16^(INK4a)蛋白作为宫颈癌早期诊断的生物标记物.
Objective: By detecting the expression of p16^(INK4a)and Notch1 gene in cervical lesions and analyzing their expression difference,to screen out the high sensitive and specific biomarkers for early diagnosis of cervical cancer.Methods: The expressed p16^(INK4a)and Notch1 protein was detected by immunohistochemistry S-P method in paraffin sections of 187 patients with cervical lesions after pathological examination,of which 43 cases were cervical cancer,26 cases were severe cervical intraepithelial neoplasia(CIN-Ⅲ),28 cases were moderate cervical intraepithelial neoplasia(CIN-Ⅱ),30 cases were mild cervical intraepithelial neoplasia(CIN-Ⅰ),34 cases were uterine leiomyoma,26 cases were chronic cervicitis. Results: The positive expressionrate of p16^(INK4a)gene in cervical cancer,CIN-Ⅲ,CIN-Ⅱ,CIN-Ⅰ,uterine leiomyoma and chronic cervicitis was 100.00%(43/43),92.31%(24/26),57.14%(16/28),53.33%(16/30),41.18%(14/34)and 30.77%(8/26)respectively,there was significant difference(P 0.05). The positive expression rate of Notch1 gene in cervical cancer,CIN-Ⅲ,CIN-Ⅱ,CIN-Ⅰ,uterine leiomyoma and chronic cervicitis was 83.72%(36/43),46.15%(12/26),57.14%(16/28),53.33%(16/30),44.12%(15/34)and 61.54%(16/26)respectively,there was significant difference(P0.05). The positive expression rates of p16^(INK4a)and Notch1 in the same type of cervical lesions had significant difference(P 0.05). Conclusion: By comparing the expression of p16^(INK4a)and Notch1 two genes in cervical lesions,the sensitivity and specificity of p16^(INK4a)gene is better than Notch1,so the p16^(INK4a)protein could be used as a biomarker for early diagnosis of cervical cancer.
