手法推拿配合电针治疗腰椎间盘突出所致坐骨神经痛的临床观察

The Clinical Effect of Massage combined with Electro Acupuncture on Sciatica Caused by Lumbar Disc Herniation

目的:观察手法推拿配合电针治疗腰椎间盘突出所致坐骨神经痛的疗效及对血清TNF-α、IL-6及MCP-1的影响。方法:将70例腰椎间盘突出所致坐骨神经痛患者随机分为治疗组和对照组,各35例。对照组给予临床常规治疗,治疗组在此基础上,给予手法推拿配合电针治疗,共治疗4周。比较两组治疗前后疼痛状况、躯体活动受限程度、生活质量及血清炎性物质的改善状况,并评估临床疗效。结果:两组治疗后腰、腿疼VAS评分、RMDO评分、WHOQOL-BREF评分、BRS-6评分、血清MCP-1、IL-6及TNF-α均明显降低(P<0.05),且治疗组的改善情况均明显优于对照组(P<0.05);治疗组治疗后总有效率明显高于对照组(P<0.05)。结论:采用手法推拿配合电针治疗腰椎间盘突出所致坐骨神经痛,能有效改善患者腰腿疼痛症状,缓解或消除局部炎症反应,促进坐骨神经功能恢复。

Objective： To observe the clinical effect of massage combined with electro acupuncture on sciatica caused by lumbar disc herniation and its effect on serum TNF-α, IL-6 and MCP-1. Methods： The 70 patients with sciatica caused by lumbar disc herniation were randomly divided into treatment and control groups （n=35）. The control group was given conventional treatment. On this basis, the treatment group was given massage with electro acupuncture treatment, with a total of 4 weeks. The treatment of pain, the degree of physical activity, quality of life and the improvement of serum inflammatory substances of the two groups were compared before and after treatment, and the clinical efficacy was evaluated. Results： After treatment, the lumbar and leg pain VAS score, RMDO score, WHOQOL-BREF score, BRS-6 score, serum MCP-1, IL-6 and TNF-α decreased significantly in the two groups （P〈0.05）, and the the treatment group showed better improvement than the control group （P〈0.05）. The treatment showed higher total efficiency rate than the control group （P〈0.05）. Conclusion： The application of massage and electro acupuncture treatment of sciatica caused by lumbar disc herniation can effectively improve the low back pain, relieve or eliminate inflammation and promote the recov- ery of sciatic nerve function.
