切断部分内括约肌对外剥内扎术治疗环状混合痔患者预后的影响

Effect of Cutting Part of the Internal Sphincter on Prognosis of Patients with Annular Mixed Hemorrhoids Undergoing Milligan-morgan Hemorrhoidectomy

目的探讨切断部分内括约肌对外剥内扎术治疗环状混合痔患者预后的影响。方法选取2014年6月—2016年11月收治的82例环状混合痔患者进行回顾性分析,按收治入院的时间先后将82例环状混合痔患者分为观察组和对照组,每组41例。对照组采用传统的外剥内扎术治疗,观察组采用外剥内扎术联合切断部分内括约肌治疗,观察并对比两组的临床效果及并发症情况。结果观察组术后临床总有效率高于对照组(P<0.01),术后疼痛时间和愈合时间明显短于对照组(P<0.05),并发症总发生率低于对照组(P<0.01)。结论切断部分内括约肌对外剥内扎术治疗环状混合痔患者临床疗效显著,术后并发症少,更有利于患者及时恢复。

Objective To investigate effect of cutting part of the internal sphincter on prognosis of patients with annular mixed hemorrhoids undergoing milligan-morgan hemorrhoidectomy. Methods Clinical data of 82 patients with mixed hemorrhoids admitted from June 2014 to November 2015 was retrospectively analyzed, and the patients were divid- ed into observation group and control group （n = 41 for each group） according to admission times. Control group was trea- ted with milligan-morgan hemorrhoidectomy, while observation group was treated with milligan-morgan hemorrhoidectomy combined with cutting part of the internal sphincter. Clinical effect and incidence rate of complication were observed and compared in two groups. Results In observation group, the total effective rate was significantly higher （P 〈 0. 01 ）, and postoperative pain and healing times were significantly shorter than those in control group （ P 〈 0. 05 ）. The total inci- dence rate of complications in observation group was significantly lower than that in control group （P 〈 0. 01 ）. Conclu- sion The cutting part of the internal sphincter on prognosis of patients with annular mixed hemorrhoids undergoing milli- gan-morgan hemorrhoidectomy can achieve good effect and less postoperative complications, and it is more conducive to recovery timely.
