2型糖尿病患者餐后晕厥的临床病例分析

CLINICAL ANALYSIS OF POSTPRANDIAL SYNCOPE IN TYPE 2 DIABETIC PATIENTS

目的探讨2型糖尿病患者餐后晕厥的临床特点及相关因素,为临床防治提供理论依据。方法2016年1—12月,选择因晕厥入院的2型糖尿病患者64例,监测期间根据饮食后是否发生晕厥,分为餐后晕厥组和餐后正常组,行血糖、动态血压、动态心电图、交感神经皮肤反应检查,对结果进行分析。结果本研究中年龄、高血压病程、血压变异性均为2型糖尿病患者餐后发生晕厥的高危因素,餐后低血糖、交感神经皮肤反应异常、心率的变异性是其相关因素。结论老年糖尿病患者发生餐后晕厥,低血糖不是唯一原因,与餐后低血压相关。年龄越大越易发生餐后晕厥,血压变异性与其呈正相关。自主神经功能障碍增加晕厥风险,交感神经皮肤反应异常、心率的变异性与其相关。

Objective To explore the clinical characteristics and related factors of postprandial syncope in pa- tients with type 2 diabetes mellitus, and to provide theoretical basis for clinical prevention and treatment. Methods From January 2016 to December 2016,64 patients with type 2 diabetes who were hospitalized for syncope were selected.During the monitoring period, according to whether the syncope occurred after eating, divided into postprandial syncope group and postprandial normal group.Blood glucose, ambulatory blood pres- sure,dynamic electrocardiogram andsympathetic skin response were checked up, and the results were ana- lyzed.Results In this study,age,hypertension,and blood pressure variability were high risk factors for post- prandial syncope in patients with type 2 diabetes mellitus. Postprandial hypoglycemia, abnormal sympathetic skin syncope,and heart rate variability were related factors.Conclusion Postprandial syncope in elderly pa- tients with diabetes mellitus is not the only cause,and is associated with postprandial hypotension.The older the more prone to postprandial syncope, blood pressure variability and its positive correlation.Autonomic dys- function increases the risk of syncope; sympathetic skin response abnormalities, and heart rate variability are associated with it.
