Furin和HPV联合检测在宫颈癌早期筛查中的应用 被引量:2

EARLY CERVICAL CANCER SCREENING WITH COMBINED DETECTION OF FURIN AND HPV-DNA

摘要 目的观察蛋白前体转化酶Furin在慢性宫颈炎、各级宫颈上皮内瘤变(CIN)及宫颈鳞状细胞癌(CSCC)中的表达差别,研究其作为宫颈癌早期筛查的临床意义。方法按照常规方法提取研究对象180例宫颈各类病变组织中的蛋白,通过Western Blot检测Furin蛋白表达情况,并行人乳头状瘤病毒分型检测。结果 180例标本中,Furin蛋白阳性表达率为65.6%,其中低级别子宫颈上皮内瘤变(CIN-L)组与慢性宫颈炎组比较差异有统计学意义(P<0.05);低级别宫颈上皮内瘤变(CIN-L)组与CIN-H组比较差异有统计学意义(P<0.05);高级别CIN-H组与CSCC组比较差异无统计学意义(P>0.05)。180例人乳头状瘤病毒(HPV)组中,高危型HPV检出率为76.1%,低危型为12.2%。高危型HPV在慢性宫颈炎组、CIN组、CSCC组检出率逐渐升高,差异有统计学意义(P<0.05),其中CIN-L组与CIN-H组比较差异无统计学意义(P>0.05)、CIN-H组与CSCC组比较差异不显著(P>0.05)。结论随着宫颈病变级别的升高Furin蛋白的表达呈增加趋势,有助于预测宫颈病变的进展情况,高危型HPV感染与Furin蛋白阳性表达呈正相关(P<0.05),二者联合检测是宫颈癌早期筛查的理想手段。 Objective To investigate the expression profile of Furin in chronic cervicitis, cervical cancer and cervical intraepithelial neoplasia, and to explore the clinical screening significance of Furin in cervical lesions.Methods Western blot method was used to measure Furin protein expressions in 180 cases of various cervical lesions.Quick nuclear hybridization method was used for detection of highrisk HPV.Results In 180 various cervical lesions,the expression rate of Furin was 65.6%.In CIN- L, the expression of Furin was significantly higher than chronic cervicitis tissue (P^0.05) ; In CIN- H,the expression of Furin was significantly higher than CIN- L (P 〈0.05).In CSCC, the expression of Furin had no significant difference from CIN - H ( P 〉0.05 ). In 180 various cervical lesion.s, high - risk HPV infected rate was 76.1 % and lowrisk HPV infected rate was 12.2%.The frequency of highrisk HPV in chronic cervici- tis,CIN and CSCC tissue was significantly higher (P〈0.05) and no difference was observed between CIN- L and CIN- H (P〉0.05).The frequency of highrisk HPV in CSCC had no significant different from CIN- H (P〉0.05).Conclusion The expression of Furin increases with the cervical lesion grade which makes it a screening test marker for cervical cancer and may help to determine the progressive potential of individual le- sions.The expression of Furin is strongly associated with highrisk HPV infection.The detection of Furin and HPV can be used as an ideal tumor index for early cervical cancer screening.
