雷火灸联合牵引治疗神经根伴椎动脉型颈椎病患者的疗效观察

Observation on the Curative Effect of Thunder-fire Moxibustion Combined with Traction in the Treatment of Cervical Spondylosis of Nerve Root and Vertebral Artery Type

目的探讨雷火灸联合牵引治疗神经根伴椎动脉型颈椎病患者的临床疗效。方法以我院软伤中心收治的76例神经根型颈椎病患者为研究对象,均予以颈椎间歇牵引,采用随机数字表分为对照组与观察组,每组38例,比较两组治疗前后眩晕、手指麻木、恶心及颈部酸痛等临床症状改善情况,使用世界卫生组织生活质量问卷(WHOQOL-100)评估患者治疗前后生活质量。结果治疗后,对照组呕吐率为26.3%,与观察组的25.1%比较均无显著差异(P>0.05),但观察组眩晕、手指麻木、颈肩酸痛等发生率均明显低于对照组(P<0.05);治疗后对照组社会交往评分(23.5±5.4)分,与观察组(24.3±5.6)分比较均无显著差异(P>0.05),但观察组躯体功能、心理功能及生活信念评分均明显高于对照组,组间差异均显著(P<0.05)。结论雷火灸联合牵引治疗神经根伴椎动脉型颈椎病疗效确切,可显著降低患者眩晕、颈肩酸痛、手指麻木等临床症状,并加强其躯体功能、社会交往、生活信念等,提高其生活质量,促进康复。

Objective To investigate the clinical effect of thunder-fire moxibustion combined with traction in the treatment of cervical spondylosis of nerve root and vertebral artery type. Methods 76 cases of patients with nerve root type cervical spondylosis treated in the rehabilitation department of our hospital were enrolled in the study. All patients were treated with intermittent cervical traction,and by the random number table method,patients were divided into the control group and the observation group,with 38 cases in each group. The relief of clinical symptoms such as dizziness,numbness of fingers,nausea and neck pain was compared between the two groups. The quality of life（ QOL） of patients was assessed by the World Health Organization Quality of Life with 100 questionnaires（ WHOQOL-100） before and after treatment. Results After treatment,there was no significant difference in the incidence of vomiting between the control group and the observation group（ 26. 3% vs 25. 1%）（ P〉0. 05）. The incidence rates of dizziness,numbness of fingers and neck pain of the observation group were significantly lower than that of the control group（ P〈0. 05）. After treatment,there was no significant difference in social interaction score between the control group and the observation group [（ 23. 5 ± 5. 4） vs（ 24. 3 ± 5. 6） ]（ P〉0. 05）,while the scores of physical function,psychological function and life belief in the observation group were significantly higher than those in the control group（ P〈0. 05）. Conclusion Thunder-fire moxibustion combined with traction is effective in the treatment of cervical spondylosis of nerve root and vertebral artery type. The treatment method can significantly reduce clinical symptoms in patients such as dizziness,neck and shoulder pain and numbness of fingers,strengthen patients＇ physical function,social interaction and belief in life,improve their quality of life,and promote recovery.
