慢性骨髓炎患者抑郁焦虑情绪及其相关因素分析

Mental health of chronic osteomyelitis patients and the related factors

目的探讨慢性骨髓炎患者焦虑和抑郁状况,并分析相关影响因素。方法对52例慢性骨髓炎患者和55例健康者进行医院焦虑抑郁量表(HADS)问卷调查和比较,包括焦虑(HADS-A)和抑郁(HADS-A)两个亚表;并对影响患者抑郁和焦虑情绪的相关因素进行分析。结果慢性骨髓炎患者HADS-A平均值为(7.69±3.70)分,其中有焦虑表现的患者共计27例(51.9%),中度以上焦虑表现11例(21.2%);HADS-D平均得分(7.13±4.19)分,其中有抑郁表现的患者28例(53.8%),中度以上抑郁表现10例(19.2%)。慢性骨髓炎患者的HADS-A和HADS-D评分都显著高于正常人群(P<0.001)。HADS-A与手术次数(P=0.037)和患者性别(P=0.047)存在显著相关性,与病程时间可能存在相关性(P=0.052);HADS-D与手术次数存在显著相关性(P=0.014)。结论慢性骨髓炎患者的焦虑及抑郁表现较正常人群更常见,多次手术、男性和长病程是影响焦虑的主要因素;多次手术是抑郁的主要危险因素。

Objective To examine the anxiety and depressive status of chronic osteomyelitis（ COM） patients,and further analyze its related factors. Methods A total of 52 Chinese patients with COM and 55 healthy controls were enrolled,who were asked to complete the Chinese version of the Hospital Anxiety and Depression Scale（ HADS）,including HADS-A and HADS-D. Factors related to mental health were investigated as well. Results The COM patients＇ average score of HADS-A was 7. 69 ± 3. 70; and 27（ 51. 9%） patients were classified as anxiety,among whom 11（ 21. 2%） patients were classified as moderate to severe anxiety. The average score of HADS-D was 7. 13 ± 4. 19; and28（ 53. 8%） patients were classified as depression,among whom 10（ 19. 7%） patients were classified as moderate to severe depression. Both the HADS-A and HADS-D scores in COM patients were significant higher than controls（ P 〈0. 001）. HADS-A scores were in significantly correlated with operation times（ P = 0. 037） and genders（ P = 0. 047）,and in marginal correlated with duration of disease（ P = 0. 052）. HADS-A scores was significantly correlated with operation times（ P = 0. 014）. Conclusion Anxiety and depression are more common in COM patients. More operation times,male and longer duration of disease are risk factors of anxiety. In addition,more operation time is a risk factor of depression as well.
