摘要
目的比较分析腹部超声与腹部X片对肠梗阻的诊断价值。方法选取我院2010年8月至2012年8月的260例疑似肠梗阻患者进行研究,分别进行腹部超声和腹部X片检查,以手术病理为标准,观察两种检查方法的准确率、敏感性、特异性、阳性预测率和阴性预测率。结果腹部超声的肠梗阻判断准确性、肠梗阻部位准确性明显高于腹部X片,具有统计学意义(P<0.05);腹部超声的误诊率(0.4%)明显低于腹部X片(19.2%),具有统计学意义(P<0.05);腹部超声的敏感性、特异性、阳性预测率和阴性预测率分别为97.6%、90.9%、97.6%和90.9%,腹部X片的敏感性、特异性、阳性预测率和阴性预测率分别为86.5%、66.7%、86.5%和66.7%。结论对疑似肠梗阻患者进行腹部超声检查可以准确的判定,减少误差发生,而且敏感性、特异性均比较高。
Objective To compare the diagnostic value of abdominal ultrasound and abdominal X-ray in intestinal obstruction. Methods From August 2010 to August 2012,260 patients with suspected intestinal obstruction were enrolled in this study. Abdominal ultrasound and abdominal X-ray were performed. The accuracy,sensitivity,specificity,positive predictive rate and negative predictive rate of the two methods were observed according to the results of surgical pathology. Results The accuracy of abdominal ultrasound on the diagnosis and the determination of location of the intestinal obstruction were significantly higher than that of abdominal X-ray( P〈0. 05). The misdiagnosis rate( 0. 4%) of abdominal ultrasound was lower than that of the abdominal X-ray( 19. 2%) with significant difference( P〈0. 05). The sensitivity,specificity,positive predictive value and negative predictive value of abdominal ultrasound were 97. 6%,90. 9%,97. 6% and 90. 9%,and the values of abdominal X-ray were 86. 5%,66. 7%,86. 5% and 66. 7%. Conclusion For patients suspected of intestinal obstruction,abdominal ultrasound can increase the accuracy of diagnosis,reduce the incidence of error,and relatively increase the sensitivity and specificity.
