摘要
目的:分析阿昔洛韦和更昔洛韦对水痘患者抗水痘-带状疱疹病毒的临床疗效。方法:应用计算机检索PUBMED、MEDLINE、EMBASE、CNKI、VIP和万方数据库,查找和比较阿昔洛韦、更昔洛韦治疗水痘患者的随机对照试验结果,检索时间均为建库至2017年5月31日止;手工检索、下载、阅读、评价纳入文献的参考文献;按纳入和排除标准确定是否进入研究,由2人独立进行纳入文献的筛选和质量评价,采用Rev Man 5.3版统计学软件进行Meta分析。结果:纳入13篇研究文献,共1 691例患者(更昔洛韦组920例,阿昔洛韦组771例),结果显示治疗水痘患者的总有效率(Ζ=5.47,P<0.01)以及疼痛消失时间(Ζ=11.85,P<0.01)、体温复常时间(Ζ=3.35,P<0.01)和结痂时间(Ζ=5.23,P<0.01)方面,经两组间比较其差异均有统计学意义;不良反应方面仅有3篇研究文献有比较,所有不良反应均较为轻微,经两组间比较其差异均无统计学意义(Ζ=1.24,P>0.05)。结论:更昔洛韦和阿昔洛韦均有显著的抗水痘病毒作用,更昔洛韦在有效率、疼痛缓解时间、体温复常时间、结痂时间等方面均更有优势;二者不良反应均较为轻微,相互比较无差异。
Objective： To analyze the clinical efficacy of of ganciclovir and acyclovir in treating anti-varicella and herpes zoster virus in patients with varicella. Methods： PUBMED, MEDLINE, CNKI, VIP and Wanfang database was used and the results of randomized controlled trials of acyclovir and more clovivir for patients with varicella were found and compared. The search time was all built up to May 3 1, 2017. According to the inclusion and exclusion criteria, screen- ing of RCT articles, data extraction and quality assessment were completed by two investigators independently. The meta- analysis was performed by using the RevMan 5.3 software. Results： Thirteen studies involving 1 691 cases were enrolled （920 cases in ganciclovir group and 771 in acyclovir group）. The results of meta-analysis showed that ganciclovir had a high response rate than acyclovir （Z= 5.47, P〈 0.01 ）, quick pain relief time （Z= 11.85, P〈 0.01 ）, short term duration of fe- ver （Z=3.35, P〈0.01） and short time to produce scabs （Z=5.23, P〈0.01）; all the differences had statistically signifi- cance. All untoward effects were slight and had no significance （Z= 1.24, P〉0.05）. Conclusion： Both ganciclovir and acyclovir have good curative effect for varicella and they were also safe, with little adverse effects, and ganciclovir had better results in response rate with quick pain relief, shorten duration of fever and short time to produce scabs.
