心力衰竭射血分数正常患者给予左西孟旦的临床效果探析

The value of levosimendan in patients with normal ejection fraction of heart failure

目的讨论左西孟旦治疗充血性心力衰竭射血分数正常患者的临床效果。方法选取2012年7月至2014年7月在该院接受治疗的心力衰竭射血分数正常患者92例分为对照组和试验组,每组46例。两组均实施心力衰竭的常规治疗,试验组则在此基础上追加左西孟旦治疗,以心功能改善情况评估临床疗效,抽取静脉血测定两组治疗前后血浆N末端B型利钠肽原(NT-proBNP)水平,并以分级小剂量多巴酚丁胺负荷试验核素心血池显影来测定两组患者静息及负荷后的左室射血分数(LVEF)、高峰射血率(PER)、高峰射血时间(TPER)等左室收缩功能指标,计算出最大变化率后运用统计学处理所有数据。结果试验组的临床总有效率为95.7%,相较于对照组的78.3%显著更高(P<0.05);两组治疗前血浆NT-proBNP水平差异无统计学意义(P>0.05),治疗后均明显降低(P<0.05),试验组的降低幅度相对更大(P<0.05);两组治疗前后LVEF、PER与TPER比较差异无统计学意义(P<0.05);治疗后试验组的LVEF、PER与TPER最大变化率均明显增高(P<0.05),其中LVEF的最大变化率还明显高于对照组(P<0.05)。结论左西孟旦有助于改善心力衰竭射血分数正常患者左心室收缩功能与心脏功能,降低血浆NT-proBNP水平,临床疗效较为理想,对促进患者康复、提升其生活质量意义重大,可进行临床推广。

Objective To analyze and discuss the clinical effect of the treatment of levosimendan in patients with congestive heart failure with normal heart failure,in order to provide guidance for clinical treatment.Methods To analyze and discuss the clinical effect of the treatment of levosimendan in patients with congestive heart failure with normal heart failure,in order to provide guidance for clinical treatment.Results The clinical total effective rate was 95.7%,compared with the control group of 78.3% significantly higher（P0.05）;the two groups before treatment were significantly lower（P0.05）,the reduction of the experimental group was significantly lower（P0.05）,the PER,PER and MVEF in the experimental group were significantly higher（P0.05）,and the maximum change rate of LVEF was significantly higher than that in control group（P0.05）.Conclusion The treatment of heart failure with levosimendan can help improve the left ventricular systolic function and cardiac function,decrease the plasma NT-proBNP level,and clinical curative effect is more ideal.It is important to promote the rehabilitation of patients and improve their life quality.
