右美托咪定对行剖宫产的妊娠高血压患者心肌的保护作用

Protective effect of dexmedetomidine on myocardium in cesarean section patients with pregnancy induced hypertension

目的探讨右美托咪定对行剖宫产的妊娠高血压患者心肌的保护作用。方法选取该院2013年10月至2015年10月行剖宫产的妊娠高血压患者112例,采用随机数字表法分为两组,对照组患者56例采用生理盐水行心肌保护,观察组患者56例采用右美托咪定行心肌保护,比较两组患者不同时间点的心肌指标,分析两组患者的手术指征、恢复情况。结果观察组患者麻醉诱导开始时、麻醉诱导结束10min、术毕时、术后12h、术后1d心率、心型脂肪酸结合蛋白、肌酸激酶同工酶、肌钙蛋白Ⅰ、缺铁修饰清蛋白低于对照组(P<0.05)。观察组患者麻醉诱导开始时、麻醉诱导结束10min、术毕时、术后12h、术后1d平均动脉压高于对照组(P<0.05)。观察组患者自主呼吸恢复时间、睁眼时间、拔管时间早于对照组(P<0.05)。结论右美托咪定对行剖宫产的妊娠高血压患者心肌具有良好的保护作用,并可加快术后恢复速度。

Objective To investigate protective effect of dexmedetomidine on myocardium in cesarean section patients with pregnancy induced hypertension.Methods A total of 112 cesarean section patients with pregnancy induced hypertension were selected in hospital from October 2013 to October 2015,who were randomly divided into two groups.56 patients treated saline for myocardial protection as control group.56 patients treated dexmedetomidine for myocardial protection as observation group.Myocardial indexes at different time points were compared between two groups.Surgical indications and recovery were analyzed in two groups.Results Heart rate,heart fatty acid binding protein,creatine kinase isoenzyme-MB,cardiac troponinⅠ,iron deficiency modified albumin at the beginning of anesthesia induction,the end of anesthesia induction 10 min,the end of operation,postoperative 12 h,postoperative 1din observation group were lower than control group（P0.05）.Mean arterial pressure at the beginning of anesthesia induction,the end of anesthesia induction 10 min,the end of operation,postoperative 12 h,postoperative 1din observation group were higher than control group（P0.05）.Spontaneous breathing recovery time,open time,extubation time in observation group were earlier than control group（P0.05）.Conclusion Dexmedetomidine has good protective effect on myocardium in cesarean section patients with pregnancy induced hypertension,which can also promote postoperative recovery.
