摘要
介绍护士主导的全科团队家访模式在老年人、妇女儿童及慢性病人等社区重点人群保健中的作用,以有利于社区护理资源的充分利用,丰富这部分人群的健康知识,在身心方面给予其帮助,提高其健康素养和生活质量。
It introduced the nurse led general practitioner home visiting model in community focused group health care for the elderly,women,children and chronic patients,in order to make full use of community nursing resources,enrich the health knowledge of the population,help them in physical and mental aspects,and improve their health literacy and quality of life.
