血浆DNA水平对慢性白血病疗效及其预后的评估价值分析 被引量:2

摘要 目的探讨血浆DNA水平检测对慢性白血病治疗效果及预后的评估价值。方法选取本院收治的慢性粒细胞白血病患者50例(CML组)、慢性淋巴细胞白血病患者70例(CLL组)及同期健康体检者50例(对照组),检测两组患者化疗前后及对照组血浆DNA水平;同时以患者1年预后结局将患者分为死亡组与存活组,比较两组入院时、化疗结束时、化疗结束后6个月时的血浆DNA水平。结果与对照组比较,CML组、CLL组化疗前后血浆DNA水平均显著升高(P<0.05);CML组加速期和急变期患者化疗后血浆DNA水平均显著高于化疗前和慢性期化疗后(P<0.05);CLL组完全缓解(CR)患者化疗后的血浆DNA水平均显著高于同组化疗前和部分缓解(PR)、未缓解(NR)患者化疗后(P<0.05);存活组不同时间点血浆DNA水平均显著低于死亡组(P<0.05)。结论检测血浆DNA水平对慢性白血病患者的化疗效果评估和预后预测有重要价值。 Objective To explore the curative effects of plasma DNA level in the treatment of chronic leukemia and the analysis of prognosis evaluation value. MeAods 50 patients with chronic myelogenous leukemia (CML group), 70 patients with chronic lymphocytic leukemia (CLL) admitted to our hospital, and 50 healthy controls in the corresponding period (control group) were selected. The plasma DNA levels in the CML group and the CLL group before and after chemotherapy and that in the control group were detected; meanwhile, the patients were divided into a death group and a survival group according to the one-year prognostic outcomes, and the plasma DNA levels at admission, at the end of chemotherapy and six months after chemotherapy between the two groups were compared. Results Compared with the control group, the plasma DNA levels in the CML group and the CLL group after chemotherapy increased significantly (P 〈 0.05); the post-chemotherapy plasma DNA levels in the CML group in acceleration period (AP) and blast crisis (BC) were significantly higher than those before chemotherapy and after chemotherapy in chronic phased 〈 0.05); the plasma DNA levels after complete remission (CR) in the CLL group were much higher than those before chemotherapy and after chemotherapy in partial remission (PR) and non-remission (NR) patients(P 〈 0.05); the plasma DNA levels at different time points in the survival group were significantly lower than those in the death group (P 〈 0.05). Conclusion The detection of plasma DNA levels has significant value in evaluating the effects of chemotherapy in the treatment of chronic leukemia and in predicting the prognosis.
