防旋股骨近端髓内钉内固定治疗高龄股骨粗隆间骨折的疗效分析

Analysis of Curative Effect of Proximal Femoral Nail Anti-rotation Internal Fixation in Treatment of Senile Femoral Intertrochanteric Fractures

目的研究防旋股骨近端髓内钉内固定治疗高龄股骨粗隆间骨折的疗效。方法选取该院2014年5月—2016年5也期间入院的62例高龄股骨粗隆间骨折患者,将其进行随机分组后,实验组采取防旋股骨近端髓内钉内固定治疗(31例),对照组则采取锁定钢板治疗(31例),对比两组患者治疗效果,并对比手术前后的情况,并对术后并发症状情况进行愈合关节功能评分比较,分析防旋股骨近端髓内钉内固定治疗方法的效果。结果经过比较后,实验组患者手术时间(43.61±10.52)min、术中出血量(143.5±32.10)m L、术后引流时间(56.8±35.20)min、下地时间(8.51±3.51)d均与对照组手术时间(80.49±10.52)min、术中出血量(256.8±34.10)m L、术后引流时间(100.4±35.10)min、下地时间(15.13±5.14)d,差异有统计学意义(P<0.05)。在并发症的观察分析上,实验组低于对照组,经比较,差异有统计学意义(P<0.05)。两组患者术后常见并发症包括褥疮、泌尿道感染、内固定松动、下肢深静脉血栓等,实验组患者并发症发生率(3.2%)显著低于对照组(22.5%)(P<0.05)。结论在临床治疗高龄股骨粗隆间骨折的过程中,应用防旋股骨近端髓内钉内固定治疗,其效果显著,在临床中值得进行推广使用。

Objective To research the curative effect of proximal femoral nail anti-rotation internal fixation in treat-ment of senile femoral intertrochanteric fractures. Methods 62 cases of senile patients with femoral intertrochanteric fractures admitted and treated in our hospital from May 2014 to May 2016 were selected and randomly divided into two groups with 31 cases in each, the experimental group were treated with proximal femoral nail anti-rotation internal fix-ation, while the control group were treated with locking plate, and the treatment effect was compared between the two groups, and the operative situation before and after surgery was compared and the effect was analyzed. Results After comparison, the differences in the operative time, intraoperative bleeding amount, postoperative draining time, time to get out of bed between the experimental group and the control group were obvious, [（43.61±10.52）min, （143.5±32.10） mL,（56.8±35.20）min, （8.51±3.51）d vs （80.49±10.52）min, （256.8±34.10）mL, （100.4±35.10）min, （15.13±5.14）d],（P〈0.05）, and the incidence rate of complications in the experimental group was lower than that in the control group, and the difference was statistically significant（3.2% vs 22.5%）（P〈0.05）. Conclusion The effect of proximal femoral nail anti-rotation internal fixation in treatment of senile femoral intertrochanteric fractures is obvious, which is worth clini-cal promotion and application.
