MRI在睾丸肿瘤中的诊断价值

Value of MRI in Diagnosis of Tumor of Testis

目的探讨磁共振成像在睾丸肿瘤中的诊断价值。方法对该院2011年7月—2016年7月15例睾丸肿瘤患者的MRI资料结合手术和病理结果进行回顾性分析。结果 15例中,精原细胞瘤6例,胚胎性癌、支持细胞瘤各1例,畸胎瘤、混合性生殖细胞肿瘤各2例,弥漫大B细胞性淋巴瘤3例。精原细胞瘤均表现为T1WI均匀等信号,T2WI均匀低信号并更低信号分隔影,增强扫描分隔强化;胚胎性癌T1WI呈均匀低信号,T2WI呈不均匀中等高信号,增强扫描有延迟强化;支持细胞瘤T1WI呈均匀等信号,T2WI呈不均匀等低信号,增强扫描明显不均匀强化;畸胎瘤T1WI和T2WI呈高、低不等混杂信号,含有脂肪和钙化,增强后见不均匀轻度强化;混合性生殖细胞肿瘤信号不均,T1WI、T2WI呈混杂高、低信号,增强后呈明显不均匀强化。弥漫大B细胞性淋巴瘤中1例T1WI呈等、稍低信号伴结节状环形稍高信号,T2WI呈不均匀等低信号伴结节状稍高信号,增强扫描病灶大部轻度强化伴小结节状显著环形强化,2例T1WI呈等低信号,T2WI呈均匀低信号,增强后轻度均匀强化。结论 MRI能准确定位睾丸病变,能清楚显示肿瘤的信号特征并做出准确的定性诊断,为临床进一步制定治疗方案提供依据。

Objective To study the value of MRI in diagnosis of tumor of testis. Methods The MRI data and operative and pathological results of 15 cases of testis tumors admitted and treated in our hospital from July 2011 to July 2016 were retrospectively analyzed. Results Of 15 cases, there were 6 cases with spermatogone tumor, 1 case with embryonal carcinomas, 1 case with supporting cell tumor, 2 cases with teratoma, 2 cases with mixed germ cell tumor and 3 cases with diffuse large b cell lymphoma tumor, and the manifestation of spermatogone tumors was T1WI homogeneous iso-intensity signal, T2WI uniformity low signal intensity and lower signal separation shadow, the manifestation of embryonal carcinoma was T1WIhomogeneous low signal intensity, T2WI inhomogeneous equal and low signal intensity, the manifestation of teratoid tumors was T1WI and T2WI showed a high and low signal, including fat and calcification, and the signal of mixed germ cell tumors was inhomogeneous, T1WI and T2WI showed a high and low signal, and the manifestation of 1 case with diffuse large b-cell lymphoma was T1WI equal and slightly low signals with tubercular and circular slightly high signal, T2WI inhomogeneous equal and low signals with tubercular slightly high signal, and the T1WI of 2 cases was equal and low signals and T2WI homogeneous low signal. Conclusion MRI can accurately locate the testis lesions and clearly show the tumor signal features and make the accurate etiologic diagnosis, which can provide basis for the further formulation of treatment plan in clinic.
