CT诊断术前结肠癌以及术后结肠癌复发的临床价值分析

Analysis of Clinical Value of CT in Diagnosis of Recurrence of Preoperative Carcinoma of Colon and Postoperative Carcinoma of Colon

目的探讨CT诊断术前结肠癌以及术后结肠癌复发的临床价值分析。方法回顾性分析2015年11月—2016年11月在该院治疗的54例结肠癌患者的临床资料,均经过手术病理切片检查确诊,术前术后均采用多排螺旋CT诊断,与病理结果进行对照,分析术前CT诊断率、术前CT分期诊断符合率、术后复发CT诊断率、术后CT分期诊断符合率。结果术前CT诊断49例(90.74%),TNM分期中,Ⅰ期12例、Ⅱ期22例、Ⅲ期11例、Ⅳ期4例,诊断符合率分别为85.71%、88.00%、100.00%、100.00%;术后复发19例(35.19%),CT诊断17例(89.47%),TNM分期中,Ⅰ期3例、Ⅱ期7例、Ⅲ期5例、Ⅳ期2例,诊断符合率分别为75.00%、87.50%、100.00%、100.00%;Ⅲ~Ⅳ期的诊断率明显高于Ⅰ~Ⅱ期,差异有统计学意义(P<0.05)。结论 CT诊断术前结肠癌以及术后结肠癌复发具有较高的临床价值,诊断准确率高,对于TNM分期也能较为准确评估,对指导临床治疗有重要的意义。

Objective To study the analysis of clinical value of CT in diagnosis of recurrence of preoperative carcinoma of colon and postoperative carcinoma of colon. Methods 54 cases of patients with carcinoma of colon treated in our hospital from November 2015 to November 2016 were selected and the confirmed by the operative pathology sections and the multi-slice spiral CT diagnosis was used before and after surgery and compare and the results were compared with those with pathological results, and the CT diagnosis rate before surgery, CT staging diagnosis coincidence rate before surgery, recurrent CT diagnosis rate after surgery and CT staging diagnosis coincidence rate after surgery were analyzed. Results Before operation, 49 cases were diagnosed by CT(90.74%), and in the TNM staging, there were 12 cases during Ⅰ stage, 22 cases during Ⅱ stage, 11 cases during Ⅲ stage, 4 cases during Ⅳ stage, and the diagnosis coincidence rate was respectively 85.71%,88.00%,100.00%,100.00%, and 19 cases were recurrent after surgery(35.19%), and 17 cases were diagnosed by CT(89.47%), in the TNM staging, there were 3 cases during Ⅰstage, 7cases during Ⅱ stage, 5 cases during Ⅲ stage and 2 cases during Ⅳ stage and the diagnosis coincidence rate was respectively 75.00%,87.50%,100.00%,100.00%, and the diagnosis rate during the Ⅲ~Ⅳ stage was obviously higher than that during the Ⅰ~Ⅱ stage, and the differences were statistically significant(P<0.05). Conclusion The clinical value of CT in diagnosis of recurrence of preoperative carcinoma of colon and postoperative carcinoma of colon is higher, and the diagnosis accuracy is high, and it can accurately evaluate the TNM staging, and it is of important significance to guiding the clinical treatment.
