宫腔填纱治疗分娩后大出血的效果对比分析

Analysis of Comparison of Effect of Intrauterine Packing with Gauze in Treatment of Massive Hemorrhage after Delivery

目的探讨宫腔填纱治疗剖宫产手术分娩后大出血的疗效观察。方法选取2016年1—12月收治76例剖宫产产后出血的患者,按照入院日期分成两组,试验组38例,对照组38例,对照组产妇行常规止血治疗治疗,试验组在对照组基础上采用宫腔填纱配合米索前列醇进行止血治疗,比较两组患者干预后产后2、24 h出血量及产后1 min新生儿Apgar评分以及手术效果。结果试验组产妇产后2、24 h平均出血量均明显低于对照组[(60.22±32.72)m L/(106.23±97.65)m L、(115.30±36.28)m L/(252.01±101.24)m L],差异有统计学意义(P<0.05);而试验组产后1 min新生儿Apgar评分明显高于对照组[(9.57±1.50)分/(9.05±1.39)分],差异有统计学意义(P<0.05)。试验组有效率、平均手术时间显著优于对照组[(97.37%)/(86.84%)、(59.64±7.12)min/(88.25±6.34)min],差异有统计学意义(P<0.05);试验组不良反应发生率为5.26%,对照组为7.89%,两组比较差异无统计学意义(P>0.05)。结论宫腔填纱配合米索前列醇在剖宫产手术分娩后大出血中的应用效果明确,止血效果佳,且其安全性高,值得临床借鉴和推广。

Objective To discuss the comparison of effect of intrauterine packing with gauze in treatment of massive hemorrhage after delivery. Methods 76 cases of patients with massive hemorrhage after cesarean section admitted and treated in our hospital from January 2016 to December 2016 were selected and divided into two groups with 38 cases in each, the control group adopted the routine hemorrhage treatment, while the test group were treated with intrauterine packing with gauze and misoprostol on the basis of the control group, and the bleeding amount at 2 h and 24 h after delivery and Apgar score at 1 min after delivery and operation effect after intervention were compared between the two groups. Results The average bleeding amount at 2 h and 24 h after delivery in the test group was obviously lower than that in the control group[(60.22±32.72)m L/(106.23±97.65)m L,(115.30±36.28)m L/(252.01±101.24)m L], and the difference was statistically significant(P<0.05), and the Apgar score at 1 min after delivery in the test group was obviously higher than that in the control group [(9.57±1.50)points/(9.05±1.39)points], and the difference was statistically significant(P<0.05), and the effective rate and average operation time in the test group were obviously better than those in the control group[(97.37%)/(86.84%),(59.64±7.12)min/(88.25±6.34)min], and the differences were statistically significant(P<0.05),and the difference in the incidence rate of adverse reactions between the test group and the control group was not statistically significant(5.26% vs 7.89%)(P >0.05). Conclusion The application effect of intrauterine packing with gauze in treatment of massive hemorrhage after delivery is definite, and the hemostatic effect is good and the safety is high, which is worth clinical reference and promotion.
