胸腔闭式引流术治疗胸腔积液的护理体会

Nursing Experience of Closed Thoracic Drainage in Treatment of Pleural Effusion

目的评价综合护理干预在胸腔积液胸腔闭式引流术治疗护理中的应用效果。方法选取2015年7月—2017年1月该院胸外科收治90例患者,采用随机数字表达法分组,常规组、干预组各45例。对照组常规护理,观察组综合护理干预。评价临床疗效,统计疼痛、康复进程指标与并发症发生情况。结果干预组引流1周期间急性疼痛发生次数、日均VAS评分、胸水完全吸收时间、胸引流管留置时间、引流后住院时间分别为(4.8±1.3)次、(2.4±1.1)分、(7.3±1.3)d、(8.5±2.0)d、(11.4±2.4)d低于常规组(8.6±2.0)次、(3.5±1.4)分、(8.0±1.5)d、(11.0±1.8)d、(13.9±3.1)d,差异有统计学意义(P<0.05)。干预组并发症合计发生率22.22%低于常规组44.44%,差异有统计学意义(P<0.05)。结论综合护理干预可改善患者预后。

Objective To evaluate the application effect of comprehensive nursing intervention in the closed thoracic drainage in treatment of pleural effusion. Methods 90 cases of patients in the department of chest surgery from July2015 to January 2017 were selected and randomly divided into two groups with 45 cases in each, the control group and the observation group respectively adopted the routine nursing and comprehensive nursing intervention, and the clinical curative effect was evaluated and the pains, recovery process indexes and occurrence of complications were counted.Results The acute pain occurrence frequency in the 1-week of drainage, average VAS score daily, complete absorption time of pleural effusion, thoracic drainage tube indwelling time and length of stay after drainage in the intervention group were lower than those in the routine group [（4.8±1.3）times,（2.4±1.1）points,（7.3±1.3）d,（8.5±2.0）d,（11.4±2.4）d vs（8.6±2.0）times,（3.5±1.4）points,（8.0±1.5）d,（11.0±1.8）d,（13.9±3.1）d], and the differences were statistically significant（P〈0.05）, and the incidence rate of complications in the intervention group was lower than that in the routine group,（22.22% vs 44.44%）, and the difference was statistically significant（P〈0.05）. Conclusion The comprehensive nursing intervention can improve the prognosis of patients.
