MRI对绝经妇女体重指数与膝关节骨关节炎软骨损伤相关性分析 被引量:4

摘要 目的研究磁共振成像(MRI)检查对绝经妇女体重指数(BMI)与膝关节骨关节炎(KAO)软骨病变损伤的相关性。方法收集梧州巿中医医院2014年10月—2016年3月收治的100例绝经妇女患者的临床资料及MRI数据,按照中国成人的BMI标准,划分观察组(体重超重和肥胖)50例和对照组(体重过低和体重正常)50例。进行膝关节MRI三维脂肪抑制质子加权(3D-PD-SPACE)、脂肪抑制快速自旋回波质子加权(PD-TSE-FS)、脂肪抑制快速自旋回波T2加权(T2-TSE-FS)、快速自旋回波T1加权(T1-TSE)获取图像,由2名高年资的医学影像医师对膝关节6个软骨面进行Recht标准分级。统计BMI与KOA软骨损伤程度、年龄与KOA软骨损伤程度均采用Spearman行相关分析。BMI与KOA软骨损伤程度行多重线性回归分析。结果观察组BMI(27.39±2.56)高于对照组BMI(21.07±2.05),差异有统计学意义(P<0.01);绝经妇女年龄越大,膝关节髌骨面(r_s=0.208,P<0.05)、股骨内侧髁面(r_s=0.267,P<0.01)的软骨Recht分级越高,软骨损伤程度越严重;绝经妇女BMI越高,膝关节髌骨面(r_s=0.227,P<0.05)、股骨滑车面(r_s=0.224,P<0.05)、股骨内侧髁面(r_s=0.498,P<0.01)、股骨外侧髁面(r_s=0.480,P<0.01)、胫骨内侧平台面(r_s=0.587,P<0.01)的软骨Recht分级越高,软骨损伤程度越严重,存在线性正相关关系。结论通过MRI对膝关节软骨损伤程度Recht分级,绝经妇女BMI与膝关节软骨损伤程度具有相关性,为临床的早期预防、准确诊断和对症治疗提供MRI诊断依据。 Objective This paper tries to investigate the correlation between the body mass index(BMI) and the lesions of knee arthritis osteoarthritis(KAO) of postmenopausal women with magnetic resonance imaging(MRI). Methods Clinical data and MRI data of 100 cases of postmenopausal women from October 2014 to March 2016 in this hospital were collected. According to the BMI standard of Chinese adults, 50 patients were in the observation group(overweight and obesity) and 50 cases were in the control group(underweight and normal weight). Knee joint MRI fat suppressed three-dimensional proton weighted(3D-PD-SPACE), fat suppressed proton weighted fast spin echo(PD-TSE-FS), fat suppressed fast spin echo T2 weighted(T2-TSE-FS), fast spin echo T1 weighted image(T1-TSE) were implemented,and two senior medical imaging physicians classified 6 cartilage surface on knee joint cartilage according to Recht classification standard. BMI, KOA cartilage damage degree, age and degree of KOA cartilage damage were analyzed by Spearman correlation analysis. BMI and KOA cartilage damage degree were analyzed by multiple linear regressions.Results The observation group BMI(27.39±2.56), higher than that of the control group BMI(21.07±2.05), the difference was statistically significant(P0.01); the older postmenopausal women were, the higher Recht degree of the knee joint patellar surface(r_s=0.208, P0.05) and the medial femoral condyle surface(r_s=0.267, P0.01), the more serious cartilage damages were; the higher BMI of postmenopausal women were, the higher Recht degree of knee joint patellar surface(r_s=0.227,P0.05), femoral trochlear surface(r_s=0.224, P0.05), the medial femoral condyle surface(r_s=0.498, P0.01),the lateral femoral condyle surface(r_s=0.480, P0.01), medial plateau surface(r_s=0.587, P0.01), the more serious the cartilage injury was, so there was a linear positive correlation. Conclusion Through the analysis of the Recht degree of knee joint damage by MRI, BMI of postmenopausal women is related to the degree of cartilage damage of knee joint. It can provide the basis of MRI diagnosis for early prevention, accurate diagnosis and symptomatic treatment.
