新型半月板双刃剪刀在半月板成形术中的应用

Application of New Meniscus Double-edged Scissors in Menisci Reformation

目的探讨新型半月板双刃剪刀在半月板成形术中的应用效果。方法将2014年12月—2016年12月间该院收治的60例半月板损伤患者随机分为对照组和观察组,每组30例。两组均行半月板成形术,对照组单采用常用半月板成形手术器械进行手术,观察组采用自制双刃剪结合常用半月板成形手术器械进行手术;比较两组手术时间、术后住院时间,比较术前术后患者红细胞计数(RBC)、白细胞计数(WBC)、血红蛋白(Hb)变化,检测并记录术前、术后1、3、6个月患者膝关节活动度(ROM)及Lysholm评分。结果观察组较对照组手术时间、术后住院时间均明显缩短,差异有统计学意义(P<0.05);观察组术前术后WBC、RBC、Hb水平比较差异无统计学意义(P>0.05),对照组术后较术前WBC、RBC、Hb发生显著变化,差异有统计学意义(P<0.05);观察组术后1、3、6个月ROM分别为(110.21±8.31)分、(130.44±6.75)分、(143.61±6.87)分,均明显大于对照组的(106.32±6.54)分、(121.64±5.84)分、(131.42±6.34)分,差异有统计学意义(P<0.05);观察组术后1、3、6个月Lysholm评分分别为(74.84±8.31)分、(82.46±6.75)分、(97.36±6.87)分均明显大于对照组的(62.35±6.54)分、(71.25±5.84)分、(84.56±5.91)分,差异有统计学意义(P<0.05)。结论半月板成形术中应用新型半月板双刃剪刀可降低手术应激反应,有效促进膝关节功能恢复,具有较高临床应用价值。

Objective This paper tries to explore the application effect of new meniscus double-edged scissors in menisci reformation. Methods From December 2014 to December 2016,60 patients with meniscus injury were randomly divided into the control group and the observation group, with 30 cases in each group. Both groups were treated with menisci reformation. The control group was treated by menisci reformation with common surgical instruments while the observation group was treated with self-made double-edged scissors combined with common surgical instruments. The surgical time, postoperative hospital stay, changes of red blood cell（RBC）, white blood cell（WBC） and hemoglobin（Hb）before and after surgery were compared between the two groups. The range of motion（ROM） of knee joint and Lysholm score were detected and recorded before surgery, one month, three months and six months after surgery. Results The surgical time and postoperative hospital stay of the observation group were significantly shorter than those of the control group,the difference was statistically significant（P0.05）. There was no significant difference in the WBC, RBC and Hb levels between the two groups（P0.05）. There were significant changes in WBC, RBC and Hb before and after surgery,the difference was statistically significant（P0.05）. ROM of the observation group of 1 month, 3 months and 6 months after surgery were（110.21±8.31）points,（130.44±6.75）points,（143.61±6.87）points, significantly higher than those of the control group of（106.32±6.54）,（121.64±5.84）points,（131.42±6.34）points,the difference was statistically significant（P0.05）, and Lysholm scores of the observation group were（74.84±8.31）points,（82.46±6.75）points,（97.36±6.87）points, also significantly higher than those of the control group of（62.35±6.54）points,（71.25±5.84）points,（84.56±5.91）points,the difference was statistically significant（P0.05）. Conclusion The use of new meniscus double-edged scissors in menisci reformation can reduce the surgical stress response and effectively promote the recovery of knee function. It is worth of clinical application.
