超声弹性成像对乳腺小结节病灶诊断价值的探讨 被引量:12

摘要 目的探讨超声弹性成像对乳腺小结节病灶良恶性的诊断价值。方法收集经彩色多普勒超声检查(彩超)发现乳腺小结节(直径0.5~1.0 cm)患者120例。患者经彩超诊断结节性质后再次行超声弹性成像检查诊断结节性质,并行结节穿刺活组织检查或手术切除。以病理结果为金标准,计算并比较彩超及超声弹性成像对乳腺结节良恶性的诊断效能。结果以病理结果为金标准,彩超及超声弹性成像诊断乳腺小结节良恶性的灵敏度分别为31%和83%,特异度分别为64%和85%,准确率分别为51%和84%。超声弹性成像诊断乳腺小结节良恶性的灵敏度、特异度及准确率均高于彩超(P均<0.01)。结论超声弹性检查对乳腺小结节良恶性具有较高的诊断价值。 Objective To investigate the application value of ultrasound elastography in the differential diagnosis of benign from malignant small breast masses.Methods Clinical data of 120 patients with breast lesions (diameter range: 0.5-1.0 cm) receiving color Doppler ultrasound examination were collected.All patients further underwent ultrasound elastography to assess the nature of the breast masses.Subsequently, the diagnosis of the breast lesions was confirmed by puncture biopsy or surgical resection.The pathological results were regarded as the gold standard.The diagnostic efficacy between color doppler ultrasound and ultrasound elastography in distinguishing benign from malignant small breast lesions was calculated and statistically compared.Results Referring to the pathological outcomes as the gold standard, the sensitivity of color Doppler ultrasound and ultrasound elastography was 31% and 83%, and the specificity was 64% and 85%, and the accurate rate was 51% and 84%. The sensitivity, specificity and accurate rate of ultrasound elastography were significantly higher compared with those of color Doppler ultrasound (all P&lt;0.01). Conclusion Ultrasound elastography is an effective approach to distinguish benign from malignant small breast lesions, which yields high sensitivity, specificity and accurate rate.
