乳腺癌肿块超声征象与术前分期的相关性研究

The Correlation research between ultrasonographic signs of breast cancer and preoperative staging

目的分析乳腺癌病灶的多普勒超声表现,探讨乳腺癌肿块超声征象与乳腺癌T分期及腋窝淋巴结转移的相关性。方法超声检查并记录90例乳腺癌肿块的大小、形态、毛刺、高回声晕、微小钙化及血流分级,以术后病理为金标准,分析记录指标与T分期及腋窝淋巴结转移的相关性。结果单因素分析结果显示乳腺癌肿块超声征象:毛刺、高回声晕、微小钙化与乳腺癌T分期有一定相关性(P<0.05),同时毛刺、微小钙化、血流分级、肿块大小与腋窝淋巴结转移有线性趋势(P<0.05),具有一定相关性(相关系数分别为R=0.305、0.236、0.439、0.446),高回声晕与腋窝淋巴结转移不存在线性趋势。结论乳腺癌肿块超声征象对评判乳腺癌T分期及腋窝淋巴结转移有重要的指导意义。

Objective To analyse the color Doppler ultrasonic features of the primary lesions of breast cancer and explore the factors related T staging and axillary lymph node metastasis（ALNM）.Methods Color Doppler ultrasound were performed in 90 breast cancer patients.We observed in turn breast cancer＇s size,boundary,internal echo,internal blood flow,hyperechoic halo and micro-calcification whether or not.Taking findings of pathological reports after operation as gold standards,Univariate analyse were conducted to determine the relation between the ultrasonic features and T staging and ALNM.Results Univariate analysis showed that there was a certain correlation between the T staging of breast cancer and the ultrasonographic sign such as burr,high echo halo,micro-calcification（p〈0.05）.At the same time,there was a linear trend in the presence of burr,micro-calcification,blood flow grading,tumor size and ALNM（p〈0.05）,and this showed some relevance（correlation coefficient R=0.324,0.219,0.308,0.412）.It could help to accurately determine the ALNM of breast cancer,and there was no linear trend between high echo halo and ALNM Conclusion The ultrasonographic signs of breast cancer have important guiding significance to judge T staging of breast cancer and ALNM.
