摘要
目的:研究芪苈强心胶囊联合果糖对慢性心力衰竭血浆心肌酶谱、总抗氧化能力及心肌灌注的影响。方法:选取我院2011年7月至2014年7月慢性心力衰竭患者106例,按照入院顺序编号,采取随机数表法将患者分为观察组与对照组两组,每组53例。对照组患者在常规治疗基础上给予果糖治疗,观察组患者在对照组治疗基础上联合芪苈强心胶囊治疗,比较两组患者临床治疗有效率以及血浆心肌酶谱、总抗氧化能力及心肌灌注指标的变化情况。结果:观察组患者临床治疗总有效率为92.45%,较对照组77.6%明显为高。两组治疗后天门冬氨酸氨基转移酶(AST)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、乳酸脱氢酶(LDH)、羟丁酸脱氢酶(HDBH)水平均较治疗前明显降低;观察组患者治疗后AST、CK、CK-MB、LDH、HDBH水平较对照组明显为低。观察组患者治疗8周后其总抗氧化能力(TAC)指标水平较对照组明显为高。两组患者LVEF、SWT指标水平较治疗前明显升高,WMI水平较治疗前明显为低;观察组患者治疗后LVEF、SWT水平较对照组明显为高,WMI水平较对照组明显为低。结论:芪苈强心胶囊联合果糖治疗慢性心力衰竭疗效显著,患者的血浆心肌酶谱水平降低,总抗氧化能力提高,心肌灌注能力得到改善,心功能得到改善,值得临床应用推广。
Objective: To study the effect of Qiliqiangxin Capsule combined with fructose on plasma myocardial enzymes,total antioxidant capacity( TAC) and myocardial perfusion in patients with chronic heart failure( CHF). Methods: 106 cases of patients with CHF treated in our hospital from July 2011 to July 2014 were selected and numbered according to the order of admission. By the random number table method,patients were divided into the observation group and the control group,53 cases in each group. Patients in two groups were treated with routine treatment and fructose while they in the observation group were addtionally treated with Qiliqiangxin Capsule. The clinical effective rate of treatment,plasma myocardial enzymes,total antioxidant capacity and myocardial perfusion indexes were compared between two groups. Results: The total effective rate of treatment in the observation group was higher than that in the control group( 92. 45% vs77. 6%)( P〈0.05). After the treatment,levels of aspartate aminotransferase( AST),creatine kinase( CK),creatine kinase isoenzyme( CK-MB),lactate dehydrogenase( LDH) and hydroxybutyrate dehydrogenase( HDBH) were significantly decreased in two groups( P〈0. 05),and levels were significantly lower in the observation group than those in the control group( P〈0. 05). After 8 weeks of treatment,TAC in the observation group was significantly higher than that in the control group( P〈0. 05),levels of LVEF and SWT in two groups were significantly higher than those before treatment( P〈0. 05) while WMI levels were significantly lower than those before treatment( P〈0. 05). After the treatment,levels of LVEF and SWT in the observation group were significantly higher than those in the control group( P〈0. 05) while WMI level was lower than that in the control group( P〈0. 05). Conclusion: Qiliqiangxin Capsule combined with fructose is significantly effective in the treatment of CHF. The regimen can decrease levels of plasma myocardial enzymes and improve the TAC,myocardial perfusion and cardiac function.
