中药疏肝健脾清热化湿法结合西药三联疗法治疗Hp相关性胃炎疗效观察

Observation of Therapeutic Effects on HP-Related Gastritis Treated with Chinese Herbal Medicine and Triple Therapy of Western Medicine

目的观察中药疏肝健脾清热化湿法结合西药三联疗法治疗幽门螺旋杆菌(Hp)相关性胃炎的临床疗效。方法以2012年8月至2014年8月Hp感染的120例胃炎患者为研究对象,随机分为治疗组和对照组,每组60例。两组患者均口服雷贝拉唑、克拉霉素和甲硝唑,治疗组加服疏肝健脾清热化湿中药,4周为1个疗程,连续服用2个疗程,比较并评价两组的临床疗效、症状积分及Hp转阴、复发率情况。结果两组患者治疗后,对照组的总有效率和Hp转阴率分别为71.6%和73.3%,治疗组分别为91.7%和93.3%,两组的总有效率和Hp转阴率比较,差异均具有统计学意义(P<0.05)。治疗组中医症状积分和白细胞介素-8(IL-8)水平均明显低于对照组(P<0.05),且两组均明显低于治疗前(P<0.05)。治疗1年后,对照组和治疗组患者的Hp复发率分别为15%和3.3%,两组比较差异具有统计学意义(P<0.05)。结论中药疏肝健脾清热化湿法结合西药三联疗法治疗Hp相关性胃炎能更有效地保护胃黏膜,抑制Hp感染,治疗胃炎效果更佳。

Objective To observe the clinical therapeutic effects on HP（ Helicobacter pylori）-related gastritis treated with Chinese herbal medicine for smoothing the liver,strengthening the spleen,clearing heat and resolving damp,combined with the triple therapy of western medicine. Methods From August 2012 to August 2014,120 patients of gastritis infected by HP were selected as the participants and randomized into a treatment group and a control group,60 cases in each one. The oral administration of radeprazole,clarithromycin and metronidazole was used. In the treatment group,Chinese herbal medicine for smoothing the liver,strengthening the spleen,clearing heat and resolving damp was used. The treatment for 4 weeks was as 1 session,continuously for 2 sessions. The clinical therapeutic effect,symptom scores and HP negative conversion and recurrence rate were compared and evaluated in the two groups. Results After treatment,the total effective rate was 71. 6% and the HP negative conversion rate was 73. 3% in the control group. Those were91. 7% and 93. 3% respectively in the treatment group. The differences in the total effective rate and HP negative conversion rate were significant statistically in comparison of the two groups（ P〈 0. 05）. TCM symptom scores and IL-8 in the treatment group were apparently lower than those in the control group（ P 〈0. 05）. The results after treatment were all lower than those before treatment in the two groups（ P 〈0. 05）.One year after treatment,HP recurrence rate was 15% in the control group and was 3. 3% in the treatment group,indicating the significant difference in comparison（ P 〈0. 05）. Conclusion The Chinese herbal medicine for smoothing the liver,strengthening the spleen,clearing heat and resolving damp,combined with the triple therapy of western medicine effectively protects gastric mucous membrane,inhibits HP infection and achieves the much better effects on gastritis.
