应用不同方法处理腹腔镜下卵巢囊肿剥除术后创面对残留卵巢功能的影响探讨

Study on Effect of Wound on the Residual Ovarian Function after the Application of Different Methods in Laparoscopic Oophorocystectomy

目的探讨应用不同方法处理腹腔镜下卵巢囊肿剥除术后创面对残留卵巢功能的影响。方法随机抽取德州市中医院妇产科自2015年1月—2016年1月收治的行腹腔镜下卵巢囊肿剥除术患者92例,分为A组(n=46)和B组(n=46)。B组患者采用电凝止血,A组采用缝合止血。对比A、B两组患者术后的激素水平、月经异常发生率以及生活质量评分。结果 A组患者的LH为(16.9±1.2)U/L、FSH为(18.3±1.2)U/L、E2为(63.8±4.2)mmol/L;B组患者的LH为(9.7±0.6)U/L、FSH为(11.2±1.1)U/L、E2为(53.8±4.1)mmol/L。月经异常发生率(4.3%)明显低于B组患者的(28.3%),A组的躯体功能评分为(63.2±3.1)分、心理功能评分为(45.9±4.2)分、社会功能评分为(61.7±4.8)分、物质生活评分为(58.6±3.7)分;B组的躯体功能评分为(51.9±4.7)分、心理功能评分为(38.6±5.2)分、社会功能评分为(52.8±4.9)分、物质生活评分为(52.1±4.5)分,A、B两组比较,差异有统计学意义(P<0.05)。结论采用缝合止血可以尽快恢复腹腔镜下卵巢囊肿剥除术患者的卵巢功能,降低月经异常发生率,改善了患者的生活质量,值得在临床应用中大力推广和使用。

Objective To study the effect of wound on the residual ovarian function after the application of different methods in laparo- scopic oophoroeystectomy. Methods 92 cases of patients with laparoseopie oophorocystectomy admitted and treated in our hospital from January 2015 to January 2016 were randomly selected and divided into two groups with 46 cases in each, the group A and group B respectively adopted the suture hemostasis and electrocautery, and the postoperative hormone level, incidence rate of abnormal men- struation and quality of life scores were compared between the group A and group B. Results The LH, FSH, E2 in the group A and in the group B were respectively （16.9±1.2）U/L, （18.3±1.2）U/L and （63.8±4.2）mmol/L and （9.7±0.6）U/L, （9.7±0.6）U/L, （11.2±1.1）U/L and （53.8±4.1）mmol/L, and the incidence rate of menoxenia was obviously lower than that in the group B, （4.3% vs 28.3%）, and the differences in the somatic function score, mental function score, social function score and material life score between the group A and group B were statistically significant[（63.2±3.1）points, （45.9±4.2）points, （61.7±4.8）points, （58.6±3.7）points vs （51.9±4.7）points, （38.6±5.2）points, （52.8±4.9）points, （52.1±4.5）points] （P〈0.05）. Conclusion The suture hemostasis can recover the ovarian function of patients with laparoscopic oophorocystectomy as soon as possible, reduce the incidence rate of menoxenia and improve the quality of life of patients, and it is worth great promotion and application in clinic.
