多排螺旋CT在肺良性结节与肺癌中的诊断效果分析 被引量:8

摘要 目的探讨多排螺旋CT扫描技术应用于肺良性结节和肺癌中的诊断效果,为临床肺部疾病的诊断和鉴别提供可行性依据。方法选取该院2013年2月—2015年8月接诊的经病理学确诊的肺良性结节患者47例(肺良性结节组)和肺癌患者45例(肺癌组),通过对两组患者进行64排螺旋CT扫描诊断,分析两组CT影像学表现及诊断效果。结果肺良性结节组疾病检出准确率(91.48%)和肺癌组检出准确率(93.33%)差异无统计学意义(P>0.05);肺良性结节组毛刺征(长毛刺81.36%、短毛刺18.60%)、阻塞性炎性反应(无81.36%、三角状11.63%、小片状6.98%)、晕圈征(有67.44%、无32.56%)、胸膜凹陷征(良性72.09%、恶性27.91%)和肺癌组均差异有统计学意义(P<0.05),两组平均直径[分别为(2.31±0.17)cm、(2.46±0.21)cm]差异无统计学意义(P>0.05)。结论 64排螺旋CT成像扫描技术在肺良性结节和肺癌的诊断中有一定检测价值,具有不同的影像学表现,同时具有较高的疾病检出率,适于肺部结节和肿瘤疾病的诊断、鉴别及预后评估。 Objective To study the effect of multi-slice spiral CT in diagnosis of lung benign nodules and lung cancer and provide feasi- ble basis for the diagnosis and identification of lung diseases in clinic. Methods 47 cases of lung benign nodules patients and 45 cases of lung cancer patients confirmed by the pathology in our hospital from February 2013 to August 2015 were selected and scanned by the 64-slice spiral CT, and the CT imaging manifestations and diagnosis effect of the two groups were analyzed. Results The differ- ence in the test accurate rate between the lung benign nodules group and lung cancer group was not statistically significant, (91.48% vs 93.33%)(P〉0.05), and the differences in the spicule sign (long thorn 81.36%, short thorn 18.60%), obstructive inflammatory re- sponse(no 81.36%, triangular 11.63%, platelet 6.98%), halo sign (had 67.44%, no 32.56%), pleural indentation sign ( benign 72.09%, malignant 27.91%) between the two groups were statistically signifieant (P〈0.05), and the difference in the average diameter between the two groups was not statistically significant[(2.31±0.17)cm, (2.46±0.21)cm](P〉0.05). Conclusion 64-slice spiral CT imaging scan in diagnosis of lung benign nodes and lung cancer has a certain test value with different imaging manifestations and higher disease test rate, which is suitable for the diagnosis, identification and prognosis evaluation of lung nodes and tumor.
