肾组织补体C1q沉积水平在糖尿病肾病的临床应用价值研究 被引量:9

摘要 目的研究糖尿病肾病患者血清及肾组织中补体C1q(complement C1q)水平在临床诊断糖尿病肾病中的应用价值。方法将我院80例糖尿病肾病患者作为研究对象,统计患者相关临床资料,在病理分型标准基础上对患者病情进行病理分型,观察不同病理类型患者的补体C1q沉积状况,并分为C1q阳性组、C1q阴性组;对比两组患者C1q临床指标检测结果、病理及不同类型补体C1q沉积差异。结果两组患者在年龄、血压、24h尿蛋白定量、血清C1q及糖尿病病程等基本资料上对比,差异不具有统计学意义(P>0.05);在Scr(serum creatinine,血清肌酐)、Alb(serum albumin,血浆白蛋白)及eGFR(estimated glomerular filtration rate,估算肾小球滤过率)方面比较,差异具有统计学意义(P<0.05);C1q随着病理等级升高,肾组织的C1q沉积率越高,差异有统计学意义(P<0.05)。结论血清中C1q水平的改变并非普遍现象,不能作为糖尿病肾病病理分期的参考指标。肾组织补体C1q沉积水平可加重糖尿病肾病患者病理分型及临床相关指标,其具有一定的临床诊断价值。 Objective To study the clinical application of complement C1q levels in sera and renal tissues for the diagnosis of diabetic nephropathy.Methods 80 cases of diabetic nephropathy patients were included in this clinical study.Based on the standard of the patient′s condition of pathological stages and relevant clinical data, the patients were categorized into C1q positive and C1q negative groups.The comparison was carried out between C1q levels in different tissues and pathological stages.Results No significant difference was found in the aspects of age, blood pressure, 24h urinary protein, serum C1q and duration of diabetes and other basic information of the two groups (P〉0.05).Levels of Scr, Alb and eGFR were significantly different (P〈0.05).And higher levels of C1q deposition were found in the group of patients with higher pathological stages (P〈0.05).Conclusion The change of serum c1q level was not a common phenomenon and could not be used as a reference index for the pathological stage of diabetic nephropathy.The level of complement C1q deposition in renal tissue can aggravate the pathological classification.The detection of C1q deposition in renal tissues has certain clinical diagnostic value for diabetic nephropathy.
