2007～2016年卫生部全血细胞计数室间质评结果分析和总结

Summary and Analysis of External Quality Assessment of Complete Blood Count from Ministry of Health from 2007 to 2016

目的通过回顾分析和总结2007年至2016年全血细胞计数室间质评(EQA)反馈结果,了解实验室10年来参加卫生部全血细胞计数室间质评的优劣状况,从而制定改进措施和完善质量管理。方法统计实验室2007年至2016年卫生部室间质评反馈的WBC、RBC、HGB、HCT、MCV、MCH、MCHC和PLT的偏倚合格率和不合格偏倚的项目及每个项目的平均偏倚,并对成绩进行汇总分析。结果 10年来,每年的EQA汇总成绩均成功,但除了2007、2008、2010、2011、2014和2016这6年的EQA平均成绩为100分外,其他4年即2009、2012、2013、2015年的EQA平均成绩分别为95、80、81.3、98.8分,这4年的偏倚不合格项目分别为HCT、MCV/HCT、MCV、MCHC/HCT、MCV、MCHC/PLT,而这些项目的偏倚合格率为90%、70%/60%、20%、40%/50%、50%、50%/90%,平均偏倚偏大的有2012年的HCT和2013年的MCV。结论加强室内质控管理,使用配套质控品,并对质控员进行培训,是做好室间质评的基石,而及时对室间质评反馈结果进行分析和总结才是改进和完善质量管理的根本。

Objective In order to improve our experimental quality management, we have retrospectively analyzed external quality assessment （EQA） data of complete blood count from Ministry of Health during 2007 to 2016 period of time.Methods We have statistically calculated the averaged, qualified and non-qualified bias of all items data （WBC、RBC、HGB、HCT、MCV、MCH、MCHC and PLT） that were collected from Ministry of Health from 2007 to 2016.Results Our results showed the averaged EQA bias are fully qualified in year of 2007, 2008, 2010, 2011, 2014 and 2016 （with scores of 100）, less qualified in year of 2009, 2012, 2013, and 2015 （with scores of 95,80,81.3,98.8 respectively）with qualified rate of HCT（90%）,MCV/HCT（70%/60%,）,MCV（20%）,MCHC/ HCT（40%/50%）,MCV（50%）,MCHC/PLT（50%/90%）.Furthermore, the averaged bias of HCT and MCV shifted in year of 2012 and 2013.Conclusion Beside maintaining the good internal quality and controls, well-trained operators, further in-time analysis of external quality assessment could radically improve experimental management.
