崇州地区二级接生医院新生儿窒息相关因素分析

Analysis on related factors of neonatal asphyxia in the second-class materinity hospitals in Chongzhou area

目的调查崇州地区二级接生医院新生儿窒息的相关因素。方法采用病例对照研究对150例新生儿窒息患儿和450例正常新生儿的临床资料进行分析。结果经过多因素Logistic回归分析显示:孕周(OR=3.342,95%CI:0.983~7.543)、出生体质量(OR=3.342,95%CI:1.673~6.341)、母亲患有贫血(OR=3.534,95%CI:0.356~3.564)、单脐动脉(OR=2.564,95%CI:0.342~4.452、第二产程延长(OR=2.200,95%CI:1.237~3.623)、羊水污染(OR=5.234,95%CI:1.452~6.743)、脐绕颈(OR=5.213,95%CI:1.233~8.734)是导致新生儿窒息的相关危险因素。结论产前因素和产时因素依旧是引起崇州地区二级接生医院发生新生儿窒息的主要原因。预防孕期发生各种并发症,做好相关检查,为分娩时做好充分准备,避免难产导致第二产程延长。

Objective To investigate the related factors of neonatal asphyxia in the second-class materinity hospitals in Chongzhou area. Methods A case-control study was conducted to analyze the clinical data of 150 neonates with neonatal asphyxia and 450 normal neonates. Results Multivariate logistic regression analysis showed that gestational week （ OR = 3. 342, 95% CI： 0. 983-7. 543 ）, birth weight （ OR = 3. 342, 95% CI.. 1. 673 -6. 341 ） , maternal anemia （ OR = 3. 534, 95 % CI. O. 356-3. 564 ） , single umbilical artery （ OR = 2. 564, 95% CI： O. 342-4. 452, prolonged second stage of labor （ OR = 2. 200, 95% CI： 1. 237 -3. 623 ）, amniotic fluid contamination （ OR = 5. 234, 95% CI： 1. 452-6. 743 ） , umbilical cord around the neck （ OR=5. 213, 95% CI： 1. 233-8. 734） were related risk faetors of neonatal asphyxia. Conclusion Antenatal and intrapartum factors are still the main causes inducing neonatal asphyxia in the second-class materinity hospitals in Chongzhou area. Preventing perinatal complications, taking corresponding exminations, making a full preparation for delivey, and avoiding prolonged second stage of labor due to difficult delivery can reduce the occurrence of neonatal asphyxia.
