鼻唇角、颏唇角改变对骨性Ⅱ类和Ⅲ类患者面容影响的三维美学评价 被引量:9

3D aesthetic evaluation of skeletal class Ⅱ and class Ⅲ subjects following Cm-Sn-Ls and Li-Sm-Pg angle change

摘要 目的:研究骨性Ⅱ类和Ⅲ类患者鼻唇角、颏唇角改变对外貌的三维审美影响,并研究正畸医生与患者的审美差异。方法:分别选取1名骨性Ⅱ类和Ⅲ类患者,拍摄其正、侧45°像,利用3DMAX软件按照其面下三分之一形态制成三维头部模型后,改变其鼻唇角和颏唇角后分别生成16个三维头模,导入iPad后选取30名正畸医生和60名正畸患者对随机排序的人头模型进行三维审美评分。结果:对于骨性Ⅱ类患者,2组评分中鼻唇角、颏唇角变化为(+15°,-15°)和原始面貌均得分最低;专业组评分中(+10°,0)、(+15°,0)得分最高,而非专业组评分中(+5°,-5°)、(+5°,-10°)得分最高(组间评分,P<0.05)。对于骨性Ⅲ类患者,2组评分中鼻唇角、颏唇角变化为(0,+15°)、(0,+10°)和原始面貌均得分最低;专业组评分中(-5°,+15°)、(-10°,+15°)得分最高,而非专业组评分中(-10°,+5°)、(-15°,0)得分最高(组间评分,P<0.05)。结论:骨性Ⅱ类和Ⅲ类患者适当代偿后的面容更受评分者喜欢,但专业组和非专业组对于代偿的具体量的审美有差异。 Objective: To study the influence of Cm-Sn-Ls and Li-Sm-Pg angle change on facial 3D aesthetics and to compare the aesthetic evaluation difference between orthodontic professionals and non-professionals. Methods: Facial photographs of a skeletal class II and a class III subject were respectively obtained and 3D head models were created by 3DMAX. Then 16 head models were obtained for each subject by changing Cm-Sn-Ls and Li-Sm-Pg angles. 30 othodontic professionals and 60 non-professionals were chosen to score the models by NRS. Results: For class II subject, the 2 groups gave lowest score to original profile, Cm-Sn-Ls and Li-Sm-Pg angle change of + 15°, -15°; the professionals gave highest score to( + 10° ,0),( + 15°,0), while non-professionals gave highest score to ( +5°, -5°) and ( +5°, - 10°). For class III subject, the 2 groups selected original profile, (0, +15°) and (0, +10°) for lowest score, and professionals selected ( -5°, + 15°) ,( - 10°, + 15°) for highest score, while non-professionals choose ( - 10°, +5°) , ( -15° ,0) for highest score( between groups, P 〈0.05). Conclusion: Both orthodontic professionals and non-professionals like the profile with appropriate compensation for skeletal class II and class III patients, but there is some aesthetic difference about the amount of compensation between the two groups.
