布地奈德与异丙托溴铵联合雾化吸入对慢性阻塞性肺疾病急性加重期肺功能、血气指标及炎症因子的影响 被引量:34

摘要 目的探讨布地奈德与异丙托溴铵联合雾化吸入对慢性阻塞性肺疾病急性加重期(AECOPD)肺功能、血气指标及炎症因子的影响。方法选取2014年1月-2016年6月本院收治的AECOPD患者84例作为研究对象,按照随机数字表法分为2组,即观察组与对照组,每组均为42例,2组患者均接受AECOPD的常规治疗,观察组在此基础上给予布地奈德混悬液与异丙托溴铵雾化液联合吸入,检测2组患者治疗前后的肺功能指标、血气指标、炎症因子。结果治疗后,观察组FEV_1、FEV_1/FVC、PEF高于对照组;观察组与对照组PaO_2、SpO_2均高于治疗前,且观察组PaO_2、SpO_2高于对照组,观察组与对照组的PaCO_2均低于治疗前,且观察组PaCO_2低于对照组;观察组和对照组的CRP、PCT均低于治疗前,且观察组低于对照组,以上差异均有统计学意义(P<0.05)。结论布地奈德与异丙托溴铵联合雾化吸入能够显著改善AECOPD患者的肺功能及血气指标,同时有效减轻炎症反应。 Objective To investigate the effects of atomized inhalation of budesonide combined with ipratropium bromide on pulmonary function,blood gas index and inflammatory factor in patients with acute exacerbation of chronic obstructive pulmonary disease( AECOPD). Methods 84 patients with AECOPD in our hospital from January 2014 to June 2016 were selected as the subjects,and they were randomly divided into the observation group and the control group,with 42 cases in each group. Two groups were treated with AECOPD conventional treatment,while the observation group were given atomized inhalation of budesonide combined with ipratropium bromide. The pulmonary function indexes, blood gas index and inflammatory factors were measured before and after treatment in the two groups. Results After treatment,pulmonary function indexes FEV_1,FEV_1/FVC and PEF in the observation group were higher than that of the control group; after treatment,PaO_2,SpO_2 in the observation group and the control group were higher than that before treatment,and PaO_2,SpO_2 in the observation group were higher than that of the control group; PaCO_2 in the observation group and the control group were lower than that before treatment,and the PaCO_2 in the observation group was lower than that of the control group; after treatment,the CRP and PCT of the two groups were lower than those before treatment,and that of the observation group were lower than that of the control group,and the differences above all were statistically significant( P〈0. 05). Conclusion Atomized inhalation of budesonide combined with ipratropium bromide can significantly improve AECOPD patients' lung function,blood gas index,and effectively reduce inflammation.
