摘要
目的评价长链非编码RNA(lncRNA)表达谱在非小细胞肺癌(NSCLC)诊断中的价值。方法系统性检索Pub Med、中国知网(CNKI)等在线数据库收集相关文献;对文献质量进行评价,采用双变量模型合并统计量,绘制综合受试者工作曲线(SROC);通过亚组分析、敏感性分析和Meta回归分析探讨异质性来源,采用Deek’s漏斗图评估发表偏移。结果共纳入符合标准的文献10篇,包含37组数据。SROC分析显示,lncRNA表达谱检测用于诊断NSCLC的灵敏度为0.76(0.70~0.81),特异度为0.85(0.79~0.89),曲线下面积(AUC)达0.87(0.84~0.90)。亚组分析表明,lncRNA表达在鳞状细胞癌中的诊断效能优于腺癌(AUC分别为0.92和0.84);lncRNA联合检测、基于血浆和亚洲人群的lncRNA分析均对NSCLC有很高的诊断价值(AUC分别为0.97、0.90和0.87)。结论 lncRNA表达谱分析在NSCLC中具有较高的诊断效能,可作为NSCLC诊断的较好指标。
Objective The evaluate the overall diagnostic efficacy of long-noncoding RNAs( lncRNAs) in the identification of non-small cell lung cancer( NSCLC). Methods Literature search was performed based on the platforms of online Pub Med and CNKI databases,etc. Methodological quality of articles was systematically assessed. The pooled analyses were enabled by using the quantitative meta-analysis model. The synthetic receiver operating curve( SROC) was drawn. Influence and univariate Meta-regression analyses were performed to trace the potential heterogeneity sources. Publication bias was judged by Deeks' funnel plot asymmetry test. Results A total of 10 articles were included,including 37 sets of data. The SROC analysis manifested that lncRNA test conferred a pooled sensitivity of 0. 76( 0. 70-0. 81) and specificity of 0. 85( 0. 79-0. 89) in identification of NSCLC,corresponding to an area under ROC curves( AUC) of 0. 87( 0. 84-0. 90). Further subgroup analysis revealed that lncRNAs reached higher accuracy in confirming squamous cell carcinoma than adenocarcinoma( AUC: 0. 92 versus0. 84). Moreover,the parallel-,plasma-and Asian-based analyses achieved promising efficacy in the management of NSCLC,in which the AUCs were estimated to 0. 97,0. 90 and 0. 87,respectively. Conclusion lncRNA signature sustains a promising diagnostic capability in confirming NSCLC,and might be recommended as complementary biomarkers for NSCLC detection.
