精神科药物引起的肝功能实验室检查异常与心肌酶谱增高的相关性

The correlation between psychiatric drug-induced liver function laboratory abnormalities and the increase of myocardial enzymes

目的分析精神科药物引起的肝功能实验室检查异常与心肌酶谱增高的相关性。方法选取精神科药物引起的肝功能实验室检查异常患者112例为甲组,检测其心肌酶谱;选取精神科住院心肌酶谱异常患者60例为乙组,检测其肝功能。对2组检测结果进行相关性分析。结果精神科药物引起的肝功能实验室检查异常与心肌酶谱天门冬氨酸氨基转移酶(AST)、α羟基丁酸脱氢酶(HBDH)、乳酸脱氢酶(LDH)及磷酸肌酸激酶(CK)增高呈正相关(P<0.01),与磷酸肌酸激酶同工酶(CKMB)无相关性(P>0.05)。结论心肌酶谱增高是由于药物损伤肝细胞引起,若无心电图异常和其他临床资料提示心肌严重受损,可按药物性肝损伤处理,减少精神科药物引起的肝损伤的发生。

Objective To analysis the correlation between Psychiatric drug-induced liver function laboratory abnormalities and the increase of myocardial enzymes. Methods 112 patients with Psychiatric drug-induced liver function laborato- ry abnormalities were selected as group A, detection of their myocardial enzymes;60 patients with psychoblastic myocardium abnormalities were selected as group B, detection of liver function. To analysis the correlation between the test results of two groups. Results There was a positive correlation between the abnormalities of liver function caused by psychiatric drugs and the abnormalities of AST, HBDH, LDH and CK （ P 〈 0.01 ）, there was no correlation with CKMB （ P 〉 0.05 ）. Conclusion Increased myocardial enzymes are due to drug-induced liver injury, if there were no ECG abnormality and other clinical data, it showed that the myocardium was severely damaged, treatment of drug-induced liver injury, reduce the incidence of liver injury caused by psychotropic drugs.
