辨证施治用药结合针灸治疗脑卒中偏瘫临床观察

Clinical observation of traditional Chinese medicine syndrome differentiation combined with acupuncture and moxibustion in treatment of stroke hemiplegia

目的观察辨证施治用药结合针灸治疗脑卒中偏瘫患者的临床疗效。方法将131例脑卒中偏瘫患者随机分为中医组66例和对照组65例,中医组给予辨证施治用药和针灸治疗加康复治疗,对照组给予康复训练。采用改良Barthel指数评定患者日常生活能力(ADL),根据简式Fugl-meyer运动功能量表对患者的运动障碍程度进行判定,并观察2组下肢运动功能和步行能力。结果治疗后中医组ADL改善状况均明显优于对照组,差异有统计学意义(P<0.05),Fugl-meyer指数的改善情况显著优于对照组,差异有统计学意义(P<0.01),下肢积分、髋关节、膝关节、踝关节积分状况均明显优于对照组,差异有统计学意义(P<0.05),步行能力分级状况均明显优于对照组,差异有统计学意义(P<0.01)。结论中药辨证施治结合针灸相对于单纯康复治疗,对脑卒中偏瘫患者更有益。

Objective To observe the clinical effect of traditional Chinese medicine syndrome differentiation combined with acupuncture and moxibustion in treatment of stroke hemiplegia. Methods 131 patients with hemiplegia were ran- domly divided into TCM group（n = 66） and control group（ n = 65 ）. TCM group was given with traditional Chinese medicine syndrome differentiation and acupuncture treatment plus rehabilitation treatment, control group was given rehabilitation train- ing. The activities of daily living（ADL） was assessed by modified Barthel index. According to the simple type Fugl-meyer motor function scale to determine the degree of movement disorder in patients, And observed the lower limb motor function and walk- ing ability of two groups. Results The improvement of ADL in TCM group was significantly better than that in control group （ P 〈 0.05 ）. The improvement of Fugl-meyer index in TCM group was significantly better than that in control group, the differ- ence was statistically significant（P 〈 0.01 ）. After treatment, the scores of lower limb points, hip joint, knee joint and ankle joint of TCM group were significantly better than those of control group,the difference was statistically significant（P 〈0.05）. After treatment, the level of walking ability of TCM group was significantly better than that of control group, the difference was statistically significant （P 〈 0.01 ）. Conclusion Compared with simple rehabilitation, traditional Chinese medicine syndrome differentiation combined with acupuncture on stroke patients with hemiplegia is more beneficial.
