老年重症胰腺炎患者参与饮食管理的临床效果评价 被引量:1

摘要 目的研究老年重症胰腺炎患者参与饮食管理的效果并进行分析。方法选取我院2015年5月~2016年5月收治的老年重症胰腺炎患者192例为观察对象,根据随机数字表法分为对照组和观察组各96例。所有患者均实施抗感染、肠内营养、维持水电解质与酸碱平衡、肝肾功能维护等对症治疗。在此基础上,对照组采用普通护理,观察组实施由患者和医护人员共同参与的饮食管理干预。对比治疗后两组患者的心理状态、病情、疼痛情况、炎症因子水平以及生活质量变化。结果治疗后观察组患者SAS、SDS评分分别为(27.7±8.1)、(18.5±7.5)分,均显著低于对照组。观察组患者的CTSI、VAS评分分别为(2.4±0.1)、(2.3±0.7)分,均显著低于对照组。观察组治疗后患者血清IL-6、IL-8水平分别为(43.5±5.5)、(2.3±5.2)ng/mL,均显著低于对照组,而IL-10水平为(61.2±6.6)ng/mL,显著高于对照组。观察组治疗后患者各项生活质量评分均显著高于对照组,差异有统计学意义(P<0.05)。结论老年重症胰腺炎患者参与饮食管理的效果显著,有利于改善患者心理状况,促进病情康复,且有效缓解炎症反应,提高生活质量。 Objective To study the effects of elderly patients with severe pancreatitis on diet management. Methods 192 patients with severe pancreatitis who were admitted in our hospital from May 2015 to May 2016 were selected as observation subjects,and all patients were divided into control group(96 cases)and observation group(96 cases) according to the random number table method.All patients were treated with anti-infection enteral nutrition maintenance water electrolyte and acid-base balancing,liver and kidney function maintenance.On this basis,the patients in control group adopted general nursing,and the patients in observation group implemented the diet management intervention by the patients and the medical staff.After treatment,the psychological state,state of an illness,pain situation,levels of inflammatory factors and changes in quality of life were reported in the two groups. Results After treatment,the SAS and SDS score in the observation group were ( 27.7 ± 8.1 ) and ( 18.5 ± 7.5 ) score respectively,and both significantly lower than that in the control group. The CTSI and VAS score of the observation group were (2.4 ± 0.1 ) and (2.3±0.7 )score respectively, both significantly lower than the control group. The serum IL-6, IL-8 level in the observation group were ( 43.5± 5.5 ) and ( 2.3± 5.2 ) ng/mL respectively, and both significantly lower than that in the control group.But the serum IL-10 level in the observation group was ( 61.2 ± 6.6 ) ng/mL,significantly higher than that in the control group.The quality of life of the patients after treatment was significantly higher than the control group,and the difference was statistically significant(P 〈 0.05). Conclusion Diet management on elderly patients with severe pancreatitis is beneficial to improve the psychological condition of patients,promote the recovery of the disease,and effectively alleviate the inflammatory response and improve the quality of life.
