中国人群MTHFR C677T基因多态性与静脉血栓症相关性的Meta分析

Association between MTHFR C677T polymorphism and venous thromboembolism in the Chinese population: a Meta-analysis

目的探讨中国人群亚甲基四氢叶酸还原酶(MTHFR)C677T基因多态性与静脉血栓症(VTE)发病风险的相关性。方法计算机检索Pub Med、CNKI和万方等数据库中中国人群MTHFR C677T基因多态性与静脉血栓症相关性研究的有关文献,应用Rev Man 5.3.5软件对符合纳入及排除标准的研究进行Meta分析。结果最终纳入文献10篇,病例组938例,对照组1 157例。各基因模型异质性均不明显(I2≤50%),均采用固定效应模型进行数据合并。基因频率对比中,突变基因T与野生基因C相比,患VTE的OR为1.53(95%CI为1.34~1.74);显性模型中,突变杂合基因型CT+突变纯合基因型TT与野生纯合基因型CC相比,患VTE的OR为1.65(95%CI为1.37~1.99);隐性模型中,TT与CC+CT相比,患VTE的OR为1.80(95%CI为1.42~2.28);共显性模型中,TT与CC相比,患VTE的OR为2.37(95%CI为1.79~3.13),CT与CC相比,患VTE的OR为1.47(95%CI为1.21~1.80)。结论在中国人群中,M THFR 677突变基因T可使VTE发病风险增加。

Objective To investigate the association between methylenetetrahydrofolate reductase（ MTHFR）C677T gene polymorphism and venous thrombosis（ VTE） risk in Chinese population. Methods Pub Med,CNKI and Wanfang Database were searched,and the literatures about association between MTHFR C677 T polymorphism and venous thrombosis were collected; Meta-analysis of the studies which met the inclusion and exclusion criteria was performed by using Rev Man 5. 3. 5 software. Results Finally,10 literatures were included,938 cases in case group and1 157 cases in control group. The heterogeneity of each gene model was not significant（ I2≤50%）,and the fixed effect model was used for data consolidation. In the gene frequency comparison,comparing the mutant gene T with the wildtype gene C,the VTE OR was 1. 53（ 95% CI： 1. 34 ~ 1. 74）; in the dominant model,comparing the mutant heterozygous genotype CT and mutant homozygous genotype TT with the wild-type homozygous genotype CC,the VTE OR was1. 65（ 95% CI： 1. 37 ~ 1. 99）; in the recessive model,comparing TT with CC and CT,the VTE OR was 1. 80（ 95% CI：1. 42 ~ 2. 28）; in the co-dominant model,comparing TT with CC,and CT with CC,the VTE OR was 2. 37（ 95% CI：1. 79 ~ 3. 13） and 1. 47（ 95% CI： 1. 21 ~ 1. 80）. Conclusion In the Chinese population,MTHFR 677 mutant gene T can increase the risk of VTE.
