摘要
目的采用静息态功能磁共振成像探讨早期帕金森病(PD)患者杏仁核功能连接(rsFC)的特点及意义。方法对29例早期PD患者(PD组)及30名健康对照者(HC组)行颅脑静息态功能磁共振扫描,选双侧杏仁核为感兴趣区,PD组与HC组间脑功能连接差异用双样本t检验,并采用国际通用的统计参数图。结果与HC组相比,早期PD组中双侧壳核、前扣带回;右中央前回、前额叶、岛叶;左胼胝体下回、苍白球与双侧杏仁核rsFC减低;右额内侧回、颞下回;左楔叶、舌回、枕下回;双侧顶上小叶、枕中回、楔前叶与双侧杏仁核rsFC增强。右额内侧回、右岛叶与帕金森病综合评分(UPDRS)呈正相关,左苍白球及右壳核与UPDRS评分呈负相关。结论早期PD患者静息态下杏仁核已存在异常功能连接,可能与PD多系统受累的复杂病理机制密切相关。
Objective To investigate the characteristics of amygdala functional connectivity( rsFC) of early Parkinson's disease( PD) patients with resting-state functional MRI. Methods 29 patients with early PD( PD group) and 30 matched healthy controls( HC) underwent resting-state f MRI scanning. Bilateral amygdalas were selected as the regions of interest( ROIs). Differences between PD patients and the controls were analyzed by two-sample t test,and data were expressed by the international generic statistical parametric mapping. Results Compared with the HC group,PD group exhibited decreased rs FCs in the bilateral putamen,anterior cingulate gyrus; right precentral gyrus,insula and prefrontal cortex; left subcallosal gyrus,globus pallidus. Increased rsFCs were observed in PD patients in the bilateral superior parietal lobule,middle occipital gyrus and precuneus; right supplementary motor cortex,inferior temporal gyrus; left cuneus,lingual gyrus,inferior occipital gyrus. rs FCs in the left globus pallidus and right putamen were negatively correlated with UPDRS score. rs FCs in the right supplementary motor cortex and insula were positively correlated with UPDRS score. Conclusion The amygdala functional connectivity in early PD patients is abnormal in resting state,which would be closely related to the multiple system involvement of PD patients.
