血清PCT和CRP对基层医院ICU重症感染患者病情评估的价值

The value of serum PCT and CRP to illness evaluation of patients with severe infection in primary hospital ICU

目的探讨血清降钙素原(PCT)和C反应蛋白(CRP)对基层医院重症监护病房(ICU)重症感染患者病情的评估价值。方法选取我院2016年4月~2017年5月收治的局部重症感染患者(局部感染组)、全身重症感染患者(全身感染组)各32例,与健康组健康志愿者32例进行对比研究。对三组均进行静脉抽血、分离血清,以免疫荧光分析法检测血清PCT浓度,免疫速率散射比浊法检测CRP浓度。观察三组的血清PCT和CRP水平,比较全身性感染组和局部感染组的阳性率。结果全身性感染组和局部感染组的血清PCT和CRP水平均明显高于健康组,差异有统计学意义(P<0.01)。全身性感染组的PCT和CRP水平显著高于局部感染组,差异有统计学意义(P<0.01)。全身性感染组的PCT阳性率(93.75%)显著高于局部感染组(62.50%),差异有统计学意义(P<0.01)。两组的CRP阳性率比较,差异无统计学意义(P>0.05)。结论血清PCT与CRP指标在基层医院ICU重症感染患者病情检测方面可提供有效参考,对早期病情的诊断有极大价值,应受到足够重视。

Objective To investigate the value of serum procalcitonin（PCT） and C-reactive protein（CRP） to illness evaluation of patients with severe infection in primary hospital intensive care unit（ICU）.Methods The subjects of this study were selected from 32 cases of patients with local severe infection（local infection group） and 32 cases of patients with systemic severe infection（systemic infection group） admitted in our hospital from April 2016 to May 2017.They were compared with the health group formed by 32 healthy volunteers.Blood was drawn and serum was isolated in the three groups to test the serum PCT concentration through immunofluorescence assay and CRP concentration through rate immune scatter turbidimetry.The serum PCT and CRP levels of the three groups were observed and the positive rates were compared between the local infection group and the systemic infection group.Results The PCT and CRP levels of the local infection group and the systemic infection group was much higher than that of the health group,with significant difference（P〈0.01）.The PCT and CRP levels of the systemic infection group was much higher than that of the local infection group,with significant difference（P〈0.01）.The PCT positive rate of the systemic infection group was 93.75%,which was significantly higher than 62.50% of the local infection group,with significant difference（P〈0.01）.There was no significant difference in the CRP positive rates between the two groups（P〈0.05）.Conclusion Serum PCT and CRP indicators can provide effective reference for patients with ICU severe infection in primary hospitals,and have great value for the diagnosis of early illness,and should be paid enough attention to.
