玻璃体腔注射康柏西普联合激光光凝治疗糖尿病性黄斑水肿的临床效果 被引量:13

摘要 目的观察玻璃体腔注射康柏西普联合激光光凝对糖尿病性黄斑水肿的效果。方法选择我院2014年1月~2016年12月收治的100例(100眼)(DME)患者作为研究对象,依照随机分组原则分成观察组与对照组各50例。观察组注射抗VEGF药物(康柏西普)联合激光光凝治疗,对照组给予单纯激光光凝治疗,对比两组治疗前后的最佳矫正视力(BCVA)、黄斑中心厚度(CMT)和安全性及注射次数。结果治疗后,两组的BCVA和CMT水平均有一定程度的改善.观察组治疗1、3、6个月BCVA和CMT水平与治疗前比较差异均有统计学意义(P<0.05);对照组治疗1、3、6个月CMT水平与治疗前比较差异均有统计学意义(P<0.05);观察组治疗1、3、6个月的改善效果显著优于对照组和本组治疗前(P<0.05);随访6个月。两组病例均没有发生视网膜脱落、晶状体损伤、眼内炎,以及色素上皮撕裂等眼部不良反应,无全身性不良反应。结论玻璃体腔注射康柏西普联合激光光凝可有效降低患者的视网膜厚度并促进其BCVA的升高,并发症较少,可作为DME临床治疗中的优选方案。 Objective To study the effect of intravitreal injection of Conbercept combined with laser photocoagulation on diabetic macular edema.Methods From January 2014 to December 2016,100 patients(100 eyes) with diabetic macular edema admitted into our hospital were selected as research objects.According to the randomized principle,they were divided into observation group and control group,with 50 cases in each group.In the observation group,injection of Conbercept,an anti-VEGF drug,combined with laser photocoagulation was applied.In the control group,only laser photocoagulation was used.The best corrected visual acuity(BCVA),central macular thickness(CMT),safety,and injection frequency before and after treatment were compared.Results After therapy,the levels of BCVA and CMT were both improved.In the observation group,1,3,and 6-month after treatment,the levels of BCVA and CMT were displayed statistical differences in comparison with those before treatment(P 0.05).These comparative differences were also displayed statistical significance in the control group(P〈0.05).The improvement after 1,3,and 6-month treatment was superior to those in the control group and before treatment(P〈0.05).No ocular adverse reaction including retinal detachment,lens injury,entophthalmia,or pigment epithelium tearing was occurred during 6-month follow up visits in both groups.No general adverse reaction was reported in the two groups.Conclusion Combination of intravitreous injection of conbercept and laser photocoagulation can effectively decrease CMT in patients with diabetic retinopathy and promote the increase of BCVA in few complications,which can be considered as the optimal regimen for diabetic macular edema in clinic.
