目的观察脑白质疏松症患者 TCD的参数变化特点,探讨 TCD检查中前循环各血管 Vm、 P I值改变与脑白质疏 松症的相关性,探讨 TCD检查中 Vm、 P I值改变与脑白质疏松症严重程度的相关性 .方 技 入 选 2 0 1 4年 1 0 月？2 0 1 6年 4 月河南省许昌市立医院神经内科住院患者 8 0例,全部经头颅 MRI检查诊断为脑白质疏松症,另选同期在本院健康体检 MRI未见异常者 60例为正常组 D两组患者的其他危险因素基本-致D并根据 Fazekas等级评分表,将 经 MRI证实的脑白 质疏松症分为 0？3 级,所有入选的患者常规行 TCD检 查 ,根据 TCD检查中前循环各血管的 Vm、 P I值的变化,探 讨 TCD 在早期发现脑白质疏松症及其严重程度分级中的意义 .结崔 单纯脑白质疏松症组前循环各血管 Vm值普遍降低, PI值普 遍升高,正常组前循环各血管 Vm、PI值正常,差异有统计学意义（ P〈0. 05乂同时比较 0？3 级脑白质疏松症患者的前循环 Vm、PI值的变化, Vm 值随分级增高而降低, P I值随分级增高而增高,差异具有统计学意义（ P〈0. 05）.结论 发 现 TCD检 查前循环各血管 Vm、PI值改变与脑白质疏松症的发生具有相关性,并可间接反映脑白质疏松症的严重程度.

Objective To observe the significance change of parameters of TCD in patients with leukoaraiosis？ discuss the correlation between the changes of Vm and PI in the anterior circulation of TCD and leukoaraiosis, and the correlation be-tween the changes of Vm and PI values and the severity of leukoaraiosis in TCD. Methods From October 2014 to April 2016 , 80 patients in neurology department of Xuchang Municipal Hospital of Henan province were diagnosed with leukoaraiosis by cranial MRI examination, at the same time, 60 cases of abnormal MRI in our hospital as for normal group. The other risk fac-tors in the two groups were basically the same, the leukoaraiosis confirmed by MRI was divided into 0^3 grades according to the Fazekas scale, all selected patients were given with routine TCD examination, based on changes in Vm and PI values of the vessels in the anterior circulation of the TCD examination, investigated the significance of TCD in the early detection of leukoaraiosis and its grading of severity. Results In simple leukoaraiosis group, the Vm value of anterior circulation decreased generally, and the PI value increased generally, in normal group of the Vm and PI values of the anterior circulation were nor-mal ,and the difference was statistically significant （P〈C〇 . 05）. Meanwhile, the changes of Vm and PI values of the anterior circulation in patients with leukoaraiosis were compared between the 0^3 groups？ the Vm value decreased with the increase of grade, and the PI value increased with the increase of grade, the difference was statistically significant （P〈C〇 . 05）. Conclu-sion The changes of Vm and PI in the blood vessels of the anterior circulation after TCD examination are related to the occur-rence of leukoaraiosis, and can indirectly reflect the severity of leukoaraiosis.
