丹参酮胶囊对创伤后骨折延迟愈合患者血清sICAM-1、sVCAM-1、BMP-2水平及微循环指标的影响 被引量:12

摘要 目的观察丹参酮胶囊对创伤后骨折延迟愈合患者血清单核细胞黏附分子(ICAM-1)、血管细胞黏附分子-1(VCAM-1)、骨形态发生蛋白2(BMP-2)及微循环指标的影响。方法将150例创伤后骨折延迟愈合患者随机分为2组,对照组75例给予常规西医治疗,观察组75例在对照组治疗基础上给予丹参酮胶囊治疗。观察2组治疗前后血清s ICAM-1、s VCAM-1、BMP-2及微循环指标变化情况,统计2组骨折愈合情况。结果 2组治疗后血清ICAM-1、VCAM-1水平及红细胞聚集指数、红细胞变形指数、红细胞电脉指数、红细胞刚性指数均显著降低(P均<0.05),而BMP-2水平显著升高(P均<0.05),且观察组上述指标改善情况均显著优于对照组(P均<0.05);观察组在治疗后2个月、3个月、6个月骨折愈合恢复情况均显著优于对照组(P均<0.05),且骨折临床愈合时间显著短于对照组(P<0.05);2组治疗期间均未发现明显不良反应。结论在西医治疗基础上加用丹参酮胶囊治疗创伤性骨折延迟愈合可缩短骨折愈合时间,提高临床愈合率,其机制可能与下调ICAM-1、VCAM-1表达,上调BMP-2表达以及改善机体局部微循环有关。 Objective It is to observe the effect of danshinone capsule on serum intercellular adhesion molecule( ICAM-1),vascular cell adhesion molecule 1( VCAM-1),BMP-2( BMP-2) and microcirculation index in the patients with posttraumatic fracture delayed healing. Methods 150 patients with post-traumatic delayed healing of fracture were randomly divided into 2 groups,the observation group( 75 cases) and control group( 75 cases),the control group was given conventional western medicine treatment,the observation group was given danshinone capsule on the basis of treatment in control group.The changes of serum ICAM-1,VCAM-1,BMP-2 and microcirculation indicators before and after treatment were observed and fracture healing was evaluated in the two groups. Results The levels of serum ICAM-1,VCAM-1 level and erythrocyte aggregation index,erythrocyte deformation index,erythrocyte electric pulse index,erythrocyte rigidity index were significantly decreased,and BMP-2 was significantly increased after treatment in both group( P〈0. 05),and the improvements of these indexes in the observation group were significantly better than those in the control group( P〈0. 05). After treatment for 2,3,6months,the fracture healing in observation group were better,while the healing time was shorter than those in the control group( P〈0. 05). No obvious adverse reactions was found during treatment stage in the two groups. Conclusion Danshinone capsule on the basis of western medicine therapy can shorten fracture healing time,improve clinical healing rate in the treatment of post-traumatic fracture delayed healing,its mechanism may be related to down-regulating the expression of ICAM-1,VCAM-1,up-regulating the expression of BMP-2 and improving local microcirculation in the body.
